About Tonsillectomy (Adult)

Under general anaesthesia, the surgeon removes both tonsils through the mouth, with no external cut. Common techniques include cold steel dissection, diathermy, coblation and harmonic scalpel, each with a different balance of bleeding control and tissue damage. The operation takes about thirty to forty-five minutes. A tonsil that is much larger on one side is sent for pathology.

Infections usually become far less frequent, and sleep-related breathing often improves when large tonsils were the cause. Adults generally have more pain afterwards than children. Bleeding, mostly in the first day or around days five to ten, is the main risk. Outcomes depend on the reason for surgery, technique and how well you follow diet and pain advice.

Expect an overnight stay and up to two weeks of throat pain, often with ear pain. Because delayed bleeding can occur, plan to stay close to a hospital for at least ten days if possible. Bring your history of infections, avoid blood-thinning painkillers unless advised, and arrange cool soft food and fluids.

Usually a good fit if

  • Recurrent tonsillitis meeting medical criteria.
  • Peritonsillar abscess (quinsy) that recurs.
  • Obstructive sleep apnoea or snoring from enlarged tonsils.
  • A suspicious tonsil asymmetry needing removal for diagnosis.

May need to wait if

  • Infrequent sore throats that do not meet criteria for surgery.
  • Bleeding disorders or blood thinners not reviewed by your doctor.
  • Active infection at the time of surgery.
  • Fear of high pain levels without a proper plan.
How to prepare
  1. Record how many tonsil infections you have had and the treatment given.
  2. Tell the team about blood thinners, aspirin and other medicines; changes are on your doctor's instruction.
  3. Follow fasting instructions for general anaesthesia and avoid ibuprofen or aspirin unless told otherwise.
  4. Plan at least two weeks near a hospital for bleeding risk and rest.
Questions to ask your surgeon
  1. How many infections have I had and does that meet your criteria?
  2. Which technique do you use and what is the bleeding risk?
  3. What pain control will I have?
  4. How long should I stay near the hospital?
  5. What do I do if I bleed at home?
Red flags when comparing hospitals
  • Tonsils are removed without recording infection history.
  • No plan for managing bleeding afterwards.
  • No pain control plan.
  • Patients are sent home on the day with no accessible emergency contact.
