About Tympanoplasty (Eardrum Repair)
Under general anaesthesia, and sometimes local, the surgeon lifts the eardrum through the ear canal or a cut behind the ear and places a graft, commonly the patient's own fascia or cartilage, to close the hole. Using a microscope or endoscope, any disease in the middle ear is also dealt with. A dissolving pack holds the graft in position.
Most grafts heal and close the perforation, which keeps water out and lowers the infection risk. Success depends on the size and site of the hole, ear health, eustachian tube function, the technique and the surgeon's experience. Hearing often improves but may not be fully restored. Risks include graft failure, dizziness and taste change.
Expect a short stay and about three weeks of keeping the ear dry, not blowing the nose, and avoiding heavy exertion and flying until cleared. Bring your hearing test and ear findings. A first check at one to two weeks is typical, with a hearing test later, so arrange this with a local ENT doctor.
Usually a good fit if
- Persistent eardrum perforation with hearing loss.
- Recurrent ear infections or discharge through a hole in the eardrum.
- Wish to swim or protect the ear from water.
- A dry ear free of active infection.
May need to wait if
- Active ear infection or discharge needing treatment first.
- Poor Eustachian tube function that may reduce graft success.
- Cholesteatoma, which needs mastoid surgery.
- Heavy smoking or upper respiratory infection around the time of surgery.
How to prepare
- Have a hearing test and ear examination, and note any discharge or infections.
- Tell the team about medicines, allergies and blood thinners; changes are on your doctor's instruction.
- Keep the ear dry before surgery and treat any active infection as directed.
- Follow fasting instructions and plan for no flying or swimming after surgery until cleared.
Questions to ask your surgeon
- Which graft and approach will you use for my perforation?
- How likely is it that the eardrum will heal and how will hearing change?
- Will hearing bones be checked or repaired?
- What are the risks to taste and inner ear?
- When can I swim and fly?
Red flags when comparing hospitals
- No hearing test or ear examination before surgery.
- The surgeon guarantees a perfect hearing result.
- No instructions about keeping the ear dry.
- Active infection is ignored.
