About Bone Marrow / Stem Cell Transplant
After high-dose chemotherapy, sometimes with radiation, stem cells are infused through a vein. In an autologous transplant they come from your own earlier collection, while in an allogeneic transplant they come from a matched donor. Stem cells may be taken from the blood, the marrow or cord blood. The choice depends on the disease, donor availability and your health.
Transplant can cure some conditions and control others, and results depend on the disease type and stage, the kind of transplant, the donor match and your age and fitness. Infection, organ complications and, with donor cells, graft-versus-host disease are the main risks. An experienced transplant centre with strong supportive care makes a notable difference.
Plan about three to four weeks in hospital, then several months staying close to the team while blood counts and immunity recover. Bring complete disease records and arrange a full-time caregiver. Make sure your haematologist at home will take over follow-up, and expect fatigue to last for months.
Usually a good fit if
- Leukaemia, lymphoma or myeloma where transplant is the planned treatment.
- Aplastic anaemia, thalassaemia, sickle cell disease or inherited marrow failure.
- A suitable donor (for donor transplants) or enough collected stem cells (for autologous transplant).
- Fitness to withstand conditioning therapy.
May need to wait if
- No suitable donor or failure to collect enough stem cells.
- Severe organ impairment limiting safety.
- Active uncontrolled infection.
- Disease status where benefit is unlikely, as judged by the transplant team.
How to prepare
- Bring diagnosis reports, marrow biopsy results and treatment history, plus donor details if known.
- Do not stop any medicine unless your doctor instructs you, and review supplements with the team.
- Expect heart, lung, liver and kidney tests, infection screening and HLA typing.
- Plan for a central line, dental check and vaccination review.
- Arrange a carer and nearby accommodation for the stay and the early months.
Questions to ask your surgeon
- Do I need an autologous or allogeneic transplant, and why?
- How is my donor matched, and what is the donor type?
- What conditioning will I receive and with what risks?
- How many transplants does your centre perform each year, and what are the outcomes for my disease?
- What is the plan if the graft does not take?
- What follow-up can be done at home?
Red flags when comparing hospitals
- No dedicated transplant unit or isolation rooms.
- No HLA typing or donor search service.
- Centre cannot share its own transplant outcomes.
- No long-term follow-up plan.
Questions about Bone Marrow / Stem Cell Transplant abroad
How long will I stay in hospital for Bone Marrow / Stem Cell Transplant?
Most patients spend about 28 nights in hospital. Full recovery takes about 365 days; your surgeon will tell you when you are fit to fly home.
What is the difference between autologous and allogeneic?
Autologous uses your own stem cells, while allogeneic uses a donor's cells and carries a risk of graft-versus-host disease but a stronger anti-cancer effect.
Where do the stem cells come from?
Most are collected from the blood after growth factor injections. Bone marrow collection under anaesthesia is used in some cases.
How long is the hospital stay?
Usually two to four weeks, longer if complications arise.
How long until my immune system recovers?
Blood counts recover in weeks, but full immune recovery often takes several months to a year or more.
