About Thyroplasty (Voice Surgery)

Under local anaesthesia with sedation, so your voice can be tested, or general anaesthesia, the surgeon makes a small cut in the neck and places a custom implant, often silicone or titanium, through a window in the thyroid cartilage. The implant pushes the cord inward. The surgeon adjusts it while you speak to obtain the best voice. Injection laryngoplasty is an alternative.

Voice often becomes stronger and clearer straight away, and swallowing problems may ease. Results depend on the cause of paralysis, the exact implant position and the surgeon's experience. Swelling can briefly change the voice, and adjustments or removal of the implant are occasionally needed. Voice therapy often improves the outcome.

Many patients stay one night. Plan to rest your voice for a few days to a week and avoid heavy lifting for around two weeks. Bring your laryngoscopy video, voice assessments and scans, and arrange speech therapy and an ENT review at home, since the voice can keep improving for some weeks.

Usually a good fit if

  • Unilateral vocal cord paralysis with a breathy voice.
  • Vocal cord weakness causing aspiration or coughing when drinking.
  • Voice not improved by voice therapy or injection.
  • Nerve recovery has been waited for or is unlikely.

May need to wait if

  • Early paralysis that may still recover, where temporary injection is often preferred first.
  • Bilateral vocal cord paralysis, which needs a different approach.
  • Bleeding disorders or blood thinners not reviewed by your doctor.
  • Active neck infection.
How to prepare
  1. Have a laryngoscopy and voice assessment, and bring your records for vocal cord movement.
  2. Tell the team about medicines and blood thinners; changes are on your doctor's instruction.
  3. Follow fasting instructions for sedation or general anaesthesia.
  4. Plan voice rest and a quiet recovery for the first week.
Questions to ask your surgeon
  1. Is thyroplasty or injection laryngoplasty better for me?
  2. Which implant do you use?
  3. Will I be awake to adjust the voice?
  4. What are the risks of airway swelling or implant movement?
  5. How is the voice reviewed afterwards?
Red flags when comparing hospitals
  • No laryngoscopy before surgery.
  • Surgery is rushed before recovery of the nerve is considered.
  • No plan for airway monitoring afterwards.
  • No voice therapist or voice assessment.
