About Submandibular Gland Excision

Under general anaesthesia, the surgeon makes a small cut in a skin crease below the jaw and removes the gland, taking care to protect nearby nerves that control the lip and tongue. Blood vessels and the duct are tied off. In some cases a drain is left for a day. Endoscopic or minimally invasive approaches are used for selected stones.

The remaining glands take over saliva production, so most people notice no dry mouth. Symptoms from the gland, such as pain with eating, resolve once it is removed. Pathology confirms the diagnosis. Risks include temporary numbness or weakness of the lower lip, tongue numbness, infection and a visible scar that fades over time.

Expect one to two nights in hospital and around two weeks before returning to usual activities. Keep the wound clean and report swelling or fever. Bring any scans or biopsy results, and arrange a wound check and, if needed, review of pathology with an ENT doctor at home.

Usually a good fit if

  • Recurrent stones or blockage of the submandibular gland.
  • Chronic infection or swelling not controlled by antibiotics or less invasive options.
  • Benign tumour of the gland.
  • Persistent pain and swelling that affect eating.

May need to wait if

  • Stones or blockage that can be treated through the duct or with sialendoscopy.
  • Active infection that needs treatment first.
  • Bleeding disorders or blood thinners not reviewed by your doctor.
  • Suspected cancer that needs wider planning.
How to prepare
  1. Bring your ultrasound, CT or sialography results and any biopsy findings.
  2. Tell the team about medicines and blood thinners; changes are on your doctor's instruction.
  3. Follow fasting instructions for general anaesthesia.
  4. Plan time for wound care and avoid heavy lifting for two weeks.
Questions to ask your surgeon
  1. Can sialendoscopy or stone removal be tried before removal?
  2. How will you protect the nerves near the gland?
  3. What risk is there of temporary lip weakness or tongue numbness?
  4. When will pathology be available?
  5. What follow-up do I need at home?
Red flags when comparing hospitals
  • Gland is removed with no imaging.
  • No discussion of nerve risks.
  • Less invasive options are not considered.
  • No drain or wound care instructions.
