About Stapedectomy / Stapedotomy
Through the ear canal and under a microscope, using local or general anaesthesia, the surgeon makes a small hole in the footplate of the stapes (stapedotomy) or removes it (stapedectomy). A tiny piston prosthesis connects the incus to the inner ear fluid, restoring sound transmission. The operation usually takes about an hour.
Most patients gain significant hearing improvement, often to a level close to normal, though results vary. Success depends on the extent of otosclerosis, surgeon experience and healing. Dizziness is common for a few days, and taste disturbance can occur. Rarely, inner ear damage leads to permanent hearing loss, which is why an experienced surgeon is important.
Plan for an overnight stay or day surgery and about three weeks of avoiding heavy lifting, straining and nose-blowing. Flying and pressure changes need the surgeon's clearance. Bring your audiogram, and arrange a repeat hearing test about six weeks later with a local audiologist or ENT doctor.
Usually a good fit if
- Conductive hearing loss from otosclerosis confirmed by audiometry.
- Air-bone gap that surgery can close.
- Hearing aid is not tolerated or not desired.
- Good general health and a healthy middle ear.
May need to wait if
- Active ear infection or perforated eardrum needing repair first.
- Only hearing ear, where the risk to hearing needs careful discussion.
- Severe sensorineural hearing loss.
- Vertigo or Ménière's disease that could be affected by surgery.
How to prepare
- Have a recent audiogram and a CT scan if advised, and bring your hearing records.
- Tell the team about medicines, allergies and blood thinners; changes are on your doctor's instruction.
- Follow fasting instructions if sedation or general anaesthesia is used.
- Avoid flying and heavy lifting for the period your surgeon advises, and plan to keep the ear dry.
Questions to ask your surgeon
- Is stapedotomy or stapedectomy better for me?
- Do you use a laser or a drill?
- What hearing gain can I expect, and what is the risk of hearing loss?
- What are the risks of dizziness or taste change?
- When can I fly and have my hearing re-tested?
Red flags when comparing hospitals
- No audiogram confirms otosclerosis before surgery.
- The risk of inner-ear hearing loss is not discussed.
- Surgery is performed on both ears at the same time.
- No guidance on flying and activity restrictions.
