About Hyoid Suspension (Sleep Apnea)

Under general anaesthesia, the surgeon makes a cut in the upper neck and secures the hyoid bone to the thyroid cartilage or the lower jaw, using sutures or a small device. This moves the tongue base and surrounding tissue forward and helps keep the airway open. It is often performed together with palate or tongue-base surgery. A sleep study and sleep endoscopy help decide whether it suits you.

Results vary, because the operation changes only one level of the airway. On its own it gives a modest improvement in snoring and apnoea, and it works best as part of a combined plan. Success depends on correct patient selection, body weight and surgeon experience. A repeat sleep study a few months later measures the actual benefit.

Expect an overnight stay and some swallowing discomfort for a week or two, with a soft diet. A small neck scar is usual. Plan to stay close for a few days. Bring your sleep study results, and arrange a follow-up sleep test with a local physician and a plan for continued CPAP use if needed.

Usually a good fit if

  • Moderate to severe obstructive sleep apnoea with tongue base obstruction.
  • CPAP not tolerated or not effective.
  • Suitable neck anatomy and no major bite abnormalities for this procedure.
  • Willingness to have a repeat sleep study to check the effect.

May need to wait if

  • Central sleep apnoea or obstruction in other areas that this surgery cannot treat.
  • Severe obesity where airway surgery alone is unlikely to help.
  • Unreviewed bleeding disorders or blood thinners.
  • Active neck infection or conditions where wound healing is poor.
How to prepare
  1. Complete a sleep study and endoscopy that confirms collapse at the tongue base.
  2. Bring your CPAP machine and tell the team about heart and lung conditions and medicines.
  3. Follow fasting instructions for general anaesthesia and avoid blood thinners only on your doctor's instruction.
  4. Plan a soft diet at home and several days of rest before travel.
Questions to ask your surgeon
  1. Is my obstruction at the tongue base, and will this treat it?
  2. Will this be combined with other airway surgery?
  3. What risks are there to swallowing and voice?
  4. How will you check success after surgery?
  5. Can I still use CPAP afterwards if needed?
Red flags when comparing hospitals
  • No sleep study or airway endoscopy is done.
  • Surgery is advised alone for any sleep apnoea.
  • The risk to swallowing is not discussed.
  • No follow-up sleep study is planned.
