About PGT-SR (Structural Rearrangements)
PGT-SR is added to an IVF cycle with ICSI. After stimulation and egg collection under sedation, eggs are fertilised and grown to blastocysts. On day five or six, a few cells are removed from each embryo with a laser and sent for chromosome analysis, usually by sequencing. Embryos are frozen while waiting, and a balanced or normal one is chosen for transfer later.
Carriers of a rearrangement often produce many unbalanced embryos, so fewer embryos may be suitable and more than one cycle can be needed. Selecting a balanced or normal embryo lowers the risk of miscarriage from the rearrangement. Success still depends on the woman's age and embryo quality, and prenatal testing may be offered afterwards.
Bring your karyotype report and any genetic counselling notes before starting. Expect to stay for roughly two to three weeks around stimulation and collection, with a separate trip for the frozen transfer unless it can be done nearer home. Ask the clinic how it will report mosaic embryos, and keep all results.
Usually a good fit if
- Carriers of a balanced translocation or inversion.
- Couples with recurrent miscarriage linked to a parental rearrangement.
- Those who had a pregnancy with an unbalanced chromosome result.
- Couples unable to conceive naturally because of a rearrangement.
May need to wait if
- Few embryos expected, since many may be unbalanced and leave none for transfer.
- Unclear karyotype that has not been confirmed.
- Medical conditions needing treatment before IVF.
- Expecting no further prenatal testing.
How to prepare
- Karyotype reports for the carrier partner, with a clear description of the rearrangement.
- Genetic counselling before the cycle, to explain the expected proportion of usable embryos.
- Hormone tests, pelvic scan, semen analysis and infection screening.
- Review medicines with your doctor and change nothing yourself.
- Plan for a later frozen transfer, which may mean a second trip.
Questions to ask your surgeon
- What is my rearrangement and what proportion of embryos are likely to be balanced?
- Does your laboratory test for my specific rearrangement?
- Can the test tell balanced carriers from normal embryos?
- What is the false positive rate and how are mosaic results handled?
- How many cycles might I need?
- What prenatal testing should I arrange at home?
Red flags when comparing hospitals
- No karyotype review before IVF.
- Generic PGT-A offered when a rearrangement needs PGT-SR.
- No genetic counsellor.
- No honest discussion of low embryo yield.
- Promises of a guaranteed healthy baby.
Questions about PGT-SR (Structural Rearrangements) abroad
Which country is cheapest for PGT-SR (Structural Rearrangements)?
The lowest typical hospital prices are in India ($6,835), Thailand ($8,250), Turkey ($9,640), out of 9 countries we compare.
Do I need PGT-SR or PGT-A?
PGT-SR is designed to detect unbalanced forms of a known rearrangement, while PGT-A only counts chromosomes. Your genetic counsellor will advise which applies.
Will many embryos be abnormal?
Often a large share of embryos from carriers are unbalanced, which is why you may need more than one cycle.
Can the test tell carrier embryos?
Some methods can, but whether to transfer a balanced carrier embryo is a decision for counselling.
Is prenatal testing still needed?
Yes, confirmatory testing is advised.
