About PGT-SR (Structural Rearrangements)

PGT-SR tests IVF embryos for unbalanced chromosome rearrangements when one parent carries a translocation or inversion. It helps select embryos more likely to give a healthy pregnancy.

PGT-SR is added to an IVF cycle with ICSI. After stimulation and egg collection under sedation, eggs are fertilised and grown to blastocysts. On day five or six, a few cells are removed from each embryo with a laser and sent for chromosome analysis, usually by sequencing. Embryos are frozen while waiting, and a balanced or normal one is chosen for transfer later.

Usually a good fit if

  • Carriers of a balanced translocation or inversion.
  • Couples with recurrent miscarriage linked to a parental rearrangement.
  • Those who had a pregnancy with an unbalanced chromosome result.
  • Couples unable to conceive naturally because of a rearrangement.

May need to wait if

  • Few embryos expected, since many may be unbalanced and leave none for transfer.
  • Unclear karyotype that has not been confirmed.
  • Medical conditions needing treatment before IVF.
  • Expecting no further prenatal testing.
How to prepare
  1. Karyotype reports for the carrier partner, with a clear description of the rearrangement.
  2. Genetic counselling before the cycle, to explain the expected proportion of usable embryos.
  3. Hormone tests, pelvic scan, semen analysis and infection screening.
  4. Review medicines with your doctor and change nothing yourself.
  5. Plan for a later frozen transfer, which may mean a second trip.
Questions to ask your surgeon
  1. What is my rearrangement and what proportion of embryos are likely to be balanced?
  2. Does your laboratory test for my specific rearrangement?
  3. Can the test tell balanced carriers from normal embryos?
  4. What is the false positive rate and how are mosaic results handled?
  5. How many cycles might I need?
  6. What prenatal testing should I arrange at home?
Red flags when comparing hospitals
  • No karyotype review before IVF.
  • Generic PGT-A offered when a rearrangement needs PGT-SR.
  • No genetic counsellor.
  • No honest discussion of low embryo yield.
  • Promises of a guaranteed healthy baby.

Some methods can, but whether to transfer a balanced carrier embryo is a decision for counselling.

Is prenatal testing still needed?

Yes, confirmatory testing is advised.
