About IMSI (Intracytoplasmic Morphologically Selected)

IMSI follows the usual IVF steps of stimulation injections and egg collection under sedation. In the laboratory, the embryologist uses a specialised microscope that magnifies sperm many times more than standard ICSI, allowing small abnormalities in the head, such as vacuoles, to be seen. The sperm judged to have the best form is injected into the egg, and embryos are then grown and transferred as in any IVF cycle.

The evidence that IMSI improves live birth rates over standard ICSI is limited and mixed, so it is best seen as an option for selected couples rather than a guaranteed upgrade. Outcomes still depend mainly on the woman's age, egg quality, sperm DNA integrity and the experience of the laboratory. Ask your doctor why IMSI is suggested in your case.

Allow about two to three weeks near the clinic for monitoring, egg collection and embryo transfer. Bring previous semen analyses and IVF records so the team can judge whether the extra selection step is reasonable. Your pregnancy test comes roughly two weeks after transfer, and your home doctor can often arrange it.

Usually a good fit if

  • Severe male factor with markedly abnormal sperm shape.
  • Repeated implantation failure or poor embryo development after earlier ICSI cycles.
  • Previous cycles where the cause of poor results was thought to be sperm related.
  • Couples with unexplained poor fertilisation who wish to try an additional selection method.

May need to wait if

  • Normal semen parameters, where high-magnification selection is unlikely to add benefit.
  • Medical conditions in the woman needing optimisation before stimulation or sedation.
  • Expectation that IMSI will guarantee higher pregnancy rates, which research has not clearly shown.
  • Untreated uterine problems affecting implantation.
How to prepare
  1. Complete semen analysis, and ask whether sperm DNA fragmentation testing is advised in your case.
  2. The woman needs hormone tests, an ultrasound and an infection screen before stimulation.
  3. Review all medicines with your doctor and change nothing on your own.
  4. Plan around two to three weeks near the clinic and bring earlier IVF and semen reports.
  5. Fast before egg collection if sedation is planned, and have an adult accompany you home.
Questions to ask your surgeon
  1. What evidence do you have that IMSI will improve my outcome compared with standard ICSI?
  2. What magnification and equipment do you use, and who performs the sperm selection?
  3. How many embryos do you plan to transfer, and what happens to the rest?
  4. Should we do sperm DNA fragmentation testing first?
  5. What is the plan if fertilisation is low or fails?
  6. Which monitoring and pregnancy tests can be done at home after transfer?
Red flags when comparing hospitals
  • The clinic promises higher success with IMSI without citing evidence or your specific indication.
  • Unable to name the magnification used or the embryologist's training.
  • IMSI is added as a routine, with no discussion of the limited evidence.
  • No clear embryo transfer policy or limit on number of embryos.
  • No written embryology report.

Questions about IMSI (Intracytoplasmic Morphologically Selected) abroad

Which country is cheapest for IMSI (Intracytoplasmic Morphologically Selected)?

The lowest typical hospital prices are in India ($7,825), Thailand ($8,983), Turkey ($10,900), out of 9 countries we compare.

How is IMSI different from ICSI?

Both inject one sperm into an egg. IMSI uses much higher magnification so the embryologist can see fine detail in the sperm head and avoid sperm with visible defects.

Does IMSI improve pregnancy rates?

Evidence is mixed. It may help some men with very abnormal sperm shape, but large studies have not shown a clear benefit for all couples, so ask your specialist for the reasoning.

Is IMSI safe for the baby?

It uses the same injection as ICSI, so the risks are similar. The extra magnification is only for selecting sperm.

Is it more uncomfortable than ICSI?

No. The difference happens in the laboratory, and your experience of stimulation and egg collection is the same.
