About PCL Reconstruction

The surgeon uses arthroscopy to remove the torn ligament, prepares tunnels in the thigh bone and shin bone, and passes a graft through them. The graft may come from your own hamstring or quadriceps tendon, or from a donor, and is fixed with screws or buttons. Some surgeons use one bundle and others two, and other injured ligaments are usually repaired in the same operation.

An isolated PCL tear often does well with rehabilitation alone, so surgery is usually reserved for higher-grade or combined injuries. Results depend on the grade of laxity, associated injuries, alignment and following the rehabilitation plan, which is slower than for an ACL repair. Some looseness can remain even after a technically good operation.

You may stay one or two nights. Plan on a brace and crutches for several weeks, careful protection against the shin sagging backwards, and physiotherapy that continues for months. Remain near the hospital until the first wound review, bring your MRI and stress X-rays, and line up a physiotherapist before you travel.

Usually a good fit if

  • Persistent knee instability or pain after a grade 3 PCL tear, particularly with other ligament injuries.
  • Failure of a rehabilitation programme for an isolated tear.
  • Active people whose sport or work requires knee stability.
  • Full knee movement and swelling controlled before surgery.

May need to wait if

  • Isolated grade 1 or 2 PCL injuries, which are usually managed with physiotherapy.
  • Advanced arthritis, where reconstruction is unlikely to help.
  • Stiff, swollen knees that need to regain movement first.
  • Active infection or inability to comply with a long rehabilitation.
How to prepare
  1. Bring MRI, stress X-rays and your clinical notes; PCL injuries often involve other ligaments.
  2. Ask your doctor about anti-inflammatory and blood-thinning medicines to review before surgery.
  3. Ask which graft will be used and whether donor tissue is planned.
  4. Do pre-surgery physiotherapy to regain full movement and thigh strength where possible.
  5. Practise using crutches and arrange a brace and home physiotherapy before leaving.
Questions to ask your surgeon
  1. Is my PCL injury isolated, or do other ligaments need treating at the same time?
  2. Which graft will you use (own tendon or donor), and why?
  3. Will you use a single- or double-bundle technique, and do you perform these regularly?
  4. How long will I be in the brace, and when can I bear weight?
  5. What is the chance the knee remains slightly loose, and what does a good outcome look like?
  6. What physiotherapy and review can be arranged at home?
Red flags when comparing hospitals
  • Reconstruction recommended for a mild isolated tear without a trial of physiotherapy.
  • The surgeon rarely performs PCL surgery, which is less common than ACL reconstruction.
  • No plan for bracing in the first weeks, which is important to protect the graft.
  • No mention of treating associated ligament or alignment problems.

Questions about PCL Reconstruction abroad

Which country is cheapest for PCL Reconstruction?

The lowest typical hospital prices are in India ($7,203), Thailand ($8,213), Turkey ($9,480), out of 9 countries we compare.

How long will I stay in hospital for PCL Reconstruction?

Most patients spend about 1 nights in hospital. Full recovery takes about 270 days; your surgeon will tell you when you are fit to fly home.

Is PCL rehabilitation different from ACL?

Yes. PCL grafts are protected from backward pull on the shin, so hamstring exercises and early deep bending are limited. Following a PCL-specific protocol matters.

Will my knee be completely stable after surgery?

Many people improve a lot, but some looseness often remains compared with a normal knee. The goal is a stable, functional knee.

When can I drive?

Driving depends on which leg is operated on and your control, generally from around four to six weeks for the right leg. Your surgeon will confirm.

When can I return to sport?

Return to cutting and contact sport is generally nine to twelve months or more after surgery, depending on progress and surgeon clearance.
