What Bilateral Total Knee Replacement patients say

★★★★★ TKR in Gurugram, saving £22k vs NHS wait
NHS waiting list was 14 months. Flew to Medanta, tier-1 Zimmer implant, 4 days in hospital, walking unassisted day 3. Total cost including flights and a companion's trip was under what a private UK consult alone would have cost.

About Bilateral Total Knee Replacement

The surgeon removes worn bone and cartilage from the thigh bone and shin bone in each knee, and fits metal and plastic implants. Spinal anaesthesia is often used. Both knees can be done in one session, which means a single recovery, or in two operations weeks to months apart, which is gentler on the heart and lungs.

Most people have much less pain and better walking in both knees. Results depend on weight, alignment, general health, and how thoroughly you follow rehabilitation. Doing both knees together carries a higher risk of clots, heart strain and blood loss, so it is chosen only for those who are fit enough after assessment.

Expect about a week in hospital after a simultaneous operation, and intensive physiotherapy from day one. Stay near your surgeon for two to three weeks, and avoid flying until cleared. You will need help at home with walking aids and stairs, so arrange a carer and physiotherapy in advance.

Usually a good fit if

  • Severe arthritis in both knees causing pain and disability, not helped by non-surgical care.
  • Knee deformity or stiffness impairing walking.
  • Fit enough for a larger operation if done together.
  • Motivated for intense rehabilitation.

May need to wait if

  • Heart, lung or kidney disease that makes simultaneous surgery risky.
  • Active infection or poorly controlled diabetes.
  • Frailty, in which staged surgery is safer.
  • Poor mobility of the hips or back that would impair rehabilitation.
How to prepare
  1. Have X-rays of both knees and standing leg alignment views, plus heart and lung assessments, since simultaneous surgery is a larger load.
  2. Review all medicines, including blood thinners, with your doctor; do not change them yourself.
  3. Control diabetes, blood pressure and weight, and treat dental or skin infections.
  4. Prepare your home with raised seats and rails, and arrange help.
  5. Ask about clot prevention and fitness to fly.
Questions to ask your surgeon
  1. Should both knees be done together or in stages for me?
  2. How many bilateral knee replacements do you do and what are the results?
  3. Which implant and technique do you use?
  4. What clot and infection prevention will I have?
  5. How long will I stay and what rehabilitation follows?
  6. What physiotherapy can I do at home?
Red flags when comparing hospitals
  • Offering simultaneous surgery without heart and lung assessment.
  • No clot prevention plan.
  • No discussion of staging.
  • No rehabilitation or follow-up plan.
