About Microfracture Surgery

During arthroscopy the surgeon cleans the damaged area back to stable cartilage, then uses a small awl to make closely spaced holes through the surface of the bone. Blood and marrow cells flow into the defect and form a clot that matures into a repair tissue called fibrocartilage. It is a quick procedure, but the rehabilitation that follows is what protects the repair.

Pain and swelling often improve, particularly for small lesions in younger patients, but the new tissue is not as durable as the original cartilage and can soften with time. Results depend on the size and site of the lesion, your age, body weight, alignment and how strictly you keep to the weight-bearing restrictions. Larger defects may do better with other cartilage procedures.

This is usually day surgery, though you may stay overnight. Plan on crutches for around six weeks, and often a motion machine or guided bending exercises, so arrange physiotherapy at home before you leave. Stay in the destination until the first wound check, bring your MRI report, and expect to wait several months before running or jumping.

Usually a good fit if

  • A focal cartilage defect, roughly under 2–4 square centimetres, with surrounding healthy cartilage.
  • Knee pain, swelling or catching that has not settled with physiotherapy and activity change.
  • Normal alignment and stable ligaments, or those that can be corrected during the same operation.
  • Motivated to follow weeks of protected weight bearing.

May need to wait if

  • Widespread arthritis or kissing lesions on both joint surfaces.
  • Very large defects, where other cartilage repair techniques are usually preferred.
  • Malalignment, ligament instability or missing meniscus that has not been addressed.
  • Not willing or able to keep weight off the joint for the weeks required.
How to prepare
  1. Bring your MRI and any earlier arthroscopy reports so the lesion size and location can be confirmed.
  2. Ask your doctor which blood thinners and anti-inflammatory medicines to review before surgery.
  3. Have routine blood tests and an anaesthetic review; stop smoking if you smoke.
  4. Practise using crutches before surgery, since you will need to limit weight bearing for weeks.
  5. Arrange physiotherapy at home and, if recommended, a continuous passive motion device.
Questions to ask your surgeon
  1. How large is my defect, and is microfracture the best option or is another cartilage technique better?
  2. Do I need alignment, ligament or meniscus treatment at the same time?
  3. How long will I need crutches and a motion machine?
  4. What does repair tissue mean for the long term compared with normal cartilage?
  5. When can I return to my sport or job, and what are the signs the repair is not working?
  6. What follow-up and physiotherapy can be arranged back home?
Red flags when comparing hospitals
  • Microfracture recommended for a large or arthritic joint surface without discussing alternatives.
  • No protocol for protected weight bearing and early motion.
  • No mention that the repair tissue differs from original cartilage.
  • The surgeon cannot show you the scan findings that justify the treatment.

Questions about Microfracture Surgery abroad

Which country is cheapest for Microfracture Surgery?

The lowest typical hospital prices are in India ($5,145), Thailand ($6,268), Turkey ($7,108), out of 9 countries we compare.

Why can't I put weight on my leg afterwards?

Loading the knee too early can disturb the clot that forms the repair tissue. Weight bearing is limited for several weeks, depending on the defect location and your surgeon's protocol.

Will the cartilage grow back normally?

No. The new tissue is fibrocartilage, which is softer and less durable than natural cartilage, but it can relieve symptoms for years in suitable patients.

Can microfracture be repeated or followed by other surgery?

Sometimes other cartilage procedures can be done later if symptoms return, although a prior microfracture may affect results. Your surgeon can advise on this.

When can I return to sport?

High-impact sport is usually not allowed for around six to twelve months, and the timing depends on your defect and progress.
