About Right Hemicolectomy (Laparoscopic)

The operation is done under general anaesthesia and takes two to three hours. The surgeon makes several small cuts and inserts a camera. The right colon, the end of the ileum and the blood vessels supplying them are divided, with the lymph nodes removed in cancer. The bowel ends are joined, and the specimen is taken out through a small cut.

Keyhole surgery usually means less pain and a quicker return of bowel function than open surgery, with equal cancer outcomes when done by an experienced team. Results depend on the condition, the stage if it is cancer, general fitness and the quality of the join. Possible complications include leakage at the join, infection and ileus.

Expect about five days in hospital, starting with fluids and gradually eating more. Plan to stay nearby for around two weeks before flying. Bring colonoscopy, biopsy and CT reports. If cancer is the reason, take the pathology report to your oncologist at home, since further treatment decisions depend on the nodes and stage.

Usually a good fit if

  • Cancer of the caecum or ascending colon.
  • Large polyps not removable by endoscopy.
  • Crohn disease involving the terminal ileum and right colon.
  • Appendiceal tumours needing right colon removal.

May need to wait if

  • Advanced cancer with spread needing a different treatment plan.
  • Severe heart or lung disease unfit for anaesthesia.
  • Dense adhesions from previous surgery that may need open surgery.
  • Severe anaemia or malnutrition needing correction.
How to prepare
  1. Colonoscopy with biopsies, CT scan and blood tests including haemoglobin.
  2. Treat anaemia with iron or transfusion planning before surgery.
  3. Review blood thinners, diabetes medicines and other tablets with your doctor; change nothing yourself.
  4. Bowel preparation and antibiotics as instructed.
  5. Improve nutrition and stop smoking where possible.
Questions to ask your surgeon
  1. Why is a right hemicolectomy needed in my case?
  2. Will you convert to open surgery if needed?
  3. How many lymph nodes will be removed and examined?
  4. What is the risk of a leak from the join?
  5. How will recovery be supported in hospital?
  6. What follow-up, scans and colonoscopy are needed at home?
Red flags when comparing hospitals
  • No staging scan for cancer.
  • No multidisciplinary review for cancer.
  • Surgeon rarely performs laparoscopic colon surgery.
  • No enhanced recovery plan.
  • No follow-up plan.
