About Laparoscopic Cholecystectomy

The operation is done under general anaesthesia and takes about forty-five to ninety minutes. The surgeon makes four small cuts, inflates the abdomen with gas and inserts a camera and instruments. The cystic duct and artery are identified using the critical view of safety, closed with clips and divided, and the gallbladder is lifted out through a small cut. Conversion to open surgery is occasionally needed.

Most people get lasting relief from gallstone pain and can eat normally, though a few have looser stools for a time. The operation is very common and generally safe, and outcomes depend on how inflamed the gallbladder is and the surgeon's experience. The main serious risk is injury to the bile duct, which is rare.

Many people leave the hospital within a day, and you can usually fly after about one to two weeks if you feel well. Walk often, and expect some shoulder-tip pain from the gas for a day or two. Bring your ultrasound and liver blood test results. Report fever, jaundice or worsening pain to your doctor at home promptly.

Usually a good fit if

  • Symptomatic gallstones with repeated biliary colic.
  • Acute or chronic cholecystitis.
  • Gallstone pancreatitis, once settled.
  • Gallbladder polyps larger than about 1 cm or other suspicious findings.

May need to wait if

  • Severe heart or lung disease unfit for general anaesthesia.
  • Uncontrolled bleeding disorders.
  • Suspected gallbladder cancer, which needs a different operation.
  • Gallstones with no symptoms, which usually do not need removal.
How to prepare
  1. Ultrasound confirms gallstones; blood tests for liver function and infection are done.
  2. MRCP or other tests are used if bile duct stones are suspected.
  3. Review blood thinners and diabetes medicines with your doctor; change nothing yourself.
  4. Fast for at least six hours before surgery.
  5. Arrange transport home and someone to stay with you for the first night.
Questions to ask your surgeon
  1. Is my pain definitely from the gallbladder?
  2. How do you reduce the risk of bile duct injury?
  3. When would you convert to open surgery?
  4. Do you do routine cholangiography?
  5. What diet advice applies afterwards?
  6. Who do I call at home if I have fever or yellow skin?
Red flags when comparing hospitals
  • Surgery offered for stones with no symptoms and without explanation.
  • No ultrasound or blood tests beforehand.
  • Surgeon who cannot describe the critical view of safety.
  • No discussion of conversion to open surgery.
  • No instructions for warning signs.
