About Hiatal Hernia Repair (Nissen Fundoplication)

The surgeon works through small incisions in the upper abdomen. The stomach is drawn back below the diaphragm, the widened opening in the diaphragm is stitched narrower, and the upper stomach is wrapped around the oesophagus to strengthen the valve that keeps acid down. A full wrap is the classic Nissen; a partial wrap is sometimes chosen to lessen swallowing side effects.

Most people get lasting relief from heartburn and regurgitation and can reduce or stop acid medicines. Some have temporary difficulty swallowing, bloating, or trouble belching or vomiting. How well it works depends on confirming reflux with testing beforehand, the size of the hernia, how well the oesophagus moves, and the surgeon's experience with this operation.

Plan for a hospital stay of a night or two and a soft diet for the first weeks. Stay near the surgeon for the first week before flying home, and arrange a follow-up with your own doctor. Bring your endoscopy, pH test and manometry reports, and a list of your current medicines.

Usually a good fit if

  • Typical heartburn or regurgitation that persists despite proper acid-suppressing medicine, or a patient who does not want lifelong medication.
  • Reflux confirmed by endoscopy and pH testing, with the oesophagus muscle working normally on manometry.
  • A large or symptomatic hiatal hernia, including a paraoesophageal hernia causing pain, vomiting or breathlessness.
  • Reflux symptoms such as night cough or throat irritation that improved with acid medicine, suggesting acid is the cause.

May need to wait if

  • Symptoms not clearly caused by reflux, or normal pH testing, because surgery rarely helps those.
  • Poor oesophageal muscle movement (such as achalasia), which usually needs a looser wrap or a different operation.
  • Severe obesity, where a different operation such as gastric bypass may control reflux better.
  • Heart or lung disease that makes general anaesthesia and keyhole gas inflation risky until optimised.
How to prepare
  1. Have an upper endoscopy, and usually a barium swallow, oesophageal manometry and pH testing, so the surgeon can confirm reflux and check oesophageal muscle function.
  2. Bring previous endoscopy, biopsy and scan reports, plus a list of your acid-reducing medicines, and ask your doctor what to do with them before surgery.
  3. Follow your surgeon's fasting instructions, usually nothing to eat for 6 to 8 hours before surgery.
  4. Some surgeons ask for a short liquid diet before surgery to shrink the liver and make keyhole access easier.
  5. Check with the surgeon when it is safe to fly home, and plan to stay near the hospital for the first week.
Questions to ask your surgeon
  1. Is a full 360-degree Nissen wrap right for me, or would a partial wrap suit my oesophageal muscle function better?
  2. Will you use a mesh to reinforce the hiatus, and why or why not for my hernia size?
  3. How many anti-reflux operations do you perform each year, and how often do you convert to open surgery?
  4. What are the chances I will have trouble swallowing or bloating, and how are they managed?
  5. How will I know the operation has worked, and what follow-up tests will you arrange?
  6. Whom can I contact at home if I vomit, cannot swallow or the wrap seems to slip?
Red flags when comparing hospitals
  • Surgery offered without endoscopy, pH testing or manometry, so reflux is never actually confirmed.
  • No explanation of wrap type, hiatus repair or the chance of swallowing difficulty and gas bloat.
  • Promises that you will never need acid medicine again or that the wrap cannot fail.
  • No clear plan for a staged return to solid food, which protects the wrap in the first weeks.

Questions about Hiatal Hernia Repair (Nissen Fundoplication) abroad

Which country is cheapest for Hiatal Hernia Repair (Nissen Fundoplication)?

The lowest typical hospital prices are in India ($8,295), Thailand ($9,578), Turkey ($11,360), out of 9 countries we compare.

How long will I stay in hospital for Hiatal Hernia Repair (Nissen Fundoplication)?

Most patients spend about 2 nights in hospital. Full recovery takes about 21 days; your surgeon will tell you when you are fit to fly home.

Will I be able to belch or vomit after a Nissen wrap?

Belching and vomiting are usually reduced, which is how the wrap controls reflux. Many people feel more bloated for a few weeks. Tell your surgeon if you retch repeatedly, as this stresses the repair.

Is swallowing difficult after surgery?

Mild difficulty swallowing is common for the first weeks because of swelling around the wrap. A soft diet and small bites help. Persistent difficulty beyond a couple of months should be reviewed.

Can a hiatal hernia come back?

Yes, hernias can recur, particularly if large, or if you strain or lift heavily early. Mesh and careful diaphragm repair reduce this risk, and your surgeon can explain the approach for your case.

Will I still need acid medicine?

Many people stop or greatly reduce medicine, but this is decided by your doctor after review. Do not change your medicines yourself.
