About Ileostomy / Colostomy Creation

The operation is done under general anaesthesia, by open or keyhole surgery, and takes one to three hours. The surgeon uses a site marked beforehand by a stoma nurse, brings a loop or the end of the bowel through a small opening and stitches it to the skin. A temporary loop stoma can be reversed later, usually after several months, once the bowel beneath has healed.

A stoma can protect a join or bowel that is healing, or replace a rectum that has been removed, and most people return to work and normal activity with good stoma care. Outcomes depend on the underlying condition and on how well the stoma is sited. Complications include skin irritation, hernia beside the stoma, blockage and leakage.

Expect about five days in hospital while a stoma nurse teaches you to empty and change the appliance. Plan to stay nearby for roughly two weeks. Bring supplies for the journey home and ask for a letter listing the type of stoma. Arrange a stoma nurse near home before you travel, and ask about the plan for reversal.

Usually a good fit if

  • Bowel cancer surgery needing a temporary or permanent stoma.
  • Protection of a low bowel join after surgery.
  • Severe inflammatory bowel disease or diverticulitis.
  • Bowel obstruction, injury or severe incontinence.

May need to wait if

  • Reversal planned too soon, since bowel and wounds need months to heal.
  • Uncontrolled infection or malnutrition needing optimisation, when time allows.
  • Serious heart or lung disease needing assessment before anaesthesia.
  • Not yet having learned stoma care and having no support at home.
How to prepare
  1. Meet the stoma nurse before surgery to mark the best site and learn about bag care.
  2. Bowel preparation if surgery is planned, as instructed.
  3. Review medicines, including blood thinners and steroids, with your doctor; change nothing yourself.
  4. Improve nutrition and stop smoking where possible.
  5. Discuss whether the stoma is temporary or permanent and when reversal might be possible.
Questions to ask your surgeon
  1. Is the stoma temporary or permanent, and when can it be reversed?
  2. Which type of stoma do you recommend and why?
  3. Who will teach me stoma care before I leave?
  4. What are the risks of dehydration, hernia or skin problems?
  5. How do I get supplies at home?
  6. What follow-up do I need at home?
Red flags when comparing hospitals
  • No stoma nurse or site marking.
  • No teaching on bag care.
  • No explanation of temporary versus permanent.
  • Surgeon cannot discuss reversal plans.
  • No advice on supplies after discharge.
