About Complex Hernia Repair

Surgery is done under general anaesthesia and may be open, laparoscopic or robotic, depending on the size of the hernia and previous operations. The surgeon frees the hernia sac, removes scar tissue, returns bowel or tissue to the abdomen and closes the defect. Mesh is placed to reinforce the wall, and sometimes the muscle layers are released to allow closure.

A well-planned repair usually relieves pain and restores the abdominal wall, but recurrence is a real risk in complex hernias and is affected by the size of the defect, smoking, obesity, diabetes and wound healing. The surgeon's experience with abdominal wall reconstruction is a major factor. Infection of the mesh and fluid collections are the main complications.

Expect about two nights in hospital. Plan to stay nearby for around two weeks, and avoid lifting heavy weights for six weeks or more. An abdominal binder may be advised. Weight loss and stopping smoking before surgery lower the risk. Bring your CT scan and previous operation reports, and ask for the mesh type to be recorded.

Usually a good fit if

  • Large, recurrent or incisional hernias with pain or enlargement.
  • Hernia with episodes of bowel obstruction or trapped contents.
  • Loss of domain or skin problems over the hernia.
  • Fit adults who have optimised their weight and diabetes.

May need to wait if

  • Current smoking, uncontrolled diabetes or very high body mass index, which increase recurrence and wound problems.
  • Active skin infection or infected mesh.
  • Severe heart or lung disease unfit for anaesthesia.
  • Unrealistic expectations of no recurrence.
How to prepare
  1. Review CT scan of the abdominal wall to define the defect and contents.
  2. Optimise weight, blood sugar and smoking, since these strongly affect healing and recurrence.
  3. Review blood thinners, diabetes medicines and other tablets with your doctor; do not change them yourself.
  4. Bowel preparation and anaesthetic assessment as advised.
  5. Plan for several weeks of limited lifting and arrange help at home.
Questions to ask your surgeon
  1. Which technique and mesh position do you recommend and why?
  2. Will the repair be open, laparoscopic or robotic?
  3. How many complex repairs do you perform?
  4. What are the risks of recurrence, wound infection and mesh problems?
  5. Do I need to lose weight or stop smoking first?
  6. What activity limits and follow-up apply at home?
Red flags when comparing hospitals
  • No CT scan before complex repair.
  • Surgeon who offers only a simple stitch repair for a large hernia.
  • No discussion of weight and smoking.
  • Mesh used without explanation of type.
  • No plan for recurrence.

How long will I stay in hospital for Complex Hernia Repair?

Most patients spend about 2 nights in hospital. Full recovery takes about 42 days; your surgeon will tell you when you are fit to fly home.

Will the hernia come back?

Complex hernias have a higher recurrence risk than simple ones. Risk is lowered by mesh, weight control and not smoking.

Modern mesh is widely used and generally safe, but mesh infection or pain can occur. Your surgeon will explain the type.

When can I lift again?

Usually after about six weeks, but it may be longer after a large repair.

Will I need a stay of several days?

Often two or more nights, longer for very large hernias.
