About Endoscopic Submucosal Dissection (ESD)
ESD is carried out under general anaesthesia or deep sedation and typically takes one to three hours, depending on the size and site of the lesion. Through the endoscope, the doctor marks the edges, injects fluid to lift the lesion, cuts around it and then dissects beneath it with a fine knife. The specimen is removed whole for pathology.
Removing the lesion in one piece lets the pathologist judge accurately whether it has been fully cleared and how deep it goes, which lowers the risk of recurrence compared with piecemeal methods. Outcomes depend on the lesion's type and depth and the operator's experience, as ESD is technically demanding. If pathology shows deeper spread, further surgery may be recommended.
Expect a hospital stay of about two days, with a liquid then soft diet at first. Plan to stay nearby for roughly a week because delayed bleeding and perforation are the main risks. Bring previous endoscopy and biopsy reports. Follow-up endoscopy is needed later, so ask for the pathology report to give to your doctor at home.
Usually a good fit if
- Early gastric cancer meeting accepted endoscopic criteria.
- Early oesophageal squamous neoplasia confined to the inner layers.
- Large colorectal lesions that cannot be removed in one piece by EMR.
- Lesions suitable for en bloc removal for accurate staging.
May need to wait if
- Evidence of deep invasion or lymph node spread, where surgery is needed.
- Uncontrolled bleeding disorder or unreviewed blood thinners.
- Severe heart or lung disease making long anaesthesia risky.
- Extensive scarring under the lesion that makes dissection unsafe.
How to prepare
- Endoscopy with magnification or ultrasound, and biopsies, to assess depth and type of lesion.
- CT scan to look for spread if cancer is confirmed.
- Review blood thinners and aspirin with your doctor, who will advise on pausing them.
- Blood tests and anaesthetic assessment; fasting as instructed.
- Bowel preparation for colon ESD.
Questions to ask your surgeon
- Does my lesion meet criteria for ESD rather than EMR or surgery?
- How many ESDs do you perform per year?
- What is your rate of complete one-piece removal?
- What are the risks of bleeding, perforation and narrowing?
- What happens if pathology shows deeper invasion?
- What follow-up endoscopy is needed at home?
Red flags when comparing hospitals
- Centre with little experience of ESD.
- No pre-procedure staging.
- No pathology service able to assess margins.
- Promises that ESD is always curative.
- No explanation of what happens if pathology is adverse.
Questions about Endoscopic Submucosal Dissection (ESD) abroad
Which country is cheapest for Endoscopic Submucosal Dissection (ESD)?
The lowest typical hospital prices are in India ($6,665), Thailand ($7,908), Turkey ($9,248), out of 9 countries we compare.
How long will I stay in hospital for Endoscopic Submucosal Dissection (ESD)?
Most patients spend about 2 nights in hospital. Full recovery takes about 14 days; your surgeon will tell you when you are fit to fly home.
How is ESD different from EMR?
ESD cuts beneath the lesion with a knife to remove even large lesions in one piece. EMR uses a snare and may remove the lesion in fragments.
Can ESD cure early cancer?
In suitable early cancers, yes, but pathology decides if more treatment is needed.
What are the main risks?
Bleeding, perforation, and narrowing of the gut after large removals. Most are treated endoscopically.
Is there any scar on the skin?
No. ESD is done through the mouth or anus.
