About Endoscopic Mucosal Resection (EMR)

EMR is done during an endoscopy or colonoscopy under sedation and takes about thirty to sixty minutes. The endoscopist injects fluid under the lesion to lift it away from the deeper muscle wall, then places a snare, a wire loop, or a band around it and removes it, sometimes in pieces. Clips may close the site. The tissue goes to pathology to confirm it is fully removed.

EMR can cure many early lesions without major surgery. Results depend on the size and type of the lesion, its depth, and the endoscopist's skill, because large lesions removed piecemeal may recur and need follow-up checks. Pathology may show that the lesion is deeper than expected, in which case surgery may be advised.

You usually stay in hospital for one night and may eat soft food soon after. Plan to stay nearby for about a week to watch for delayed bleeding or perforation, the main risks. Avoid blood thinners only as your doctor advises. Surveillance endoscopy is typically repeated in months, which you can arrange at home with the pathology report.

Usually a good fit if

  • Large flat polyps in the colon that are suitable for endoscopic removal.
  • Early superficial oesophageal or stomach lesions, such as Barrett's changes.
  • Lesions confined to the inner layers on imaging.
  • People who prefer to avoid surgery where it is appropriate.

May need to wait if

  • Lesions suspected of invading deeper layers, where surgery or ESD may be better.
  • Uncorrected clotting problems or blood thinners not yet reviewed by the doctor.
  • Active infection or severe uncontrolled heart or lung disease.
  • Lesions in places where endoscopic removal is unsafe, such as areas with scarring.
How to prepare
  1. Review blood thinners and aspirin with your doctor, who will tell you if and when to pause them.
  2. Bowel preparation for colon EMR, or fasting for upper EMR, as instructed.
  3. Blood tests including clotting and haemoglobin.
  4. Arrange someone to take you home after sedation.
  5. Share past scans or biopsy results.
Questions to ask your surgeon
  1. Is EMR or ESD better for my lesion?
  2. Will the lesion be removed in one piece or pieces?
  3. What are the risks of bleeding and perforation?
  4. What will pathology tell us and what happens if cancer is found?
  5. How soon can I fly?
  6. What surveillance endoscopy do I need and when?
Red flags when comparing hospitals
  • No pre-procedure assessment of the lesion size or depth.
  • Endoscopist with little experience in therapeutic endoscopy.
  • No pathology review planned.
  • No clear plan for managing bleeding after the procedure.
  • Advice to restart blood thinners without clear instructions.
