À propos de Hépatectomie (résection hépatique)

The surgeon plans the operation from CT or MRI, deciding between removing a small wedge, one or more segments, or a whole lobe. Open and laparoscopic approaches exist, and ultrasound during surgery helps find small tumours and avoid important vessels. If the planned remnant is small, portal vein embolisation may be used first to encourage growth of the healthy side.

The outcome depends on the type and number of tumours, the health of the liver tissue, and whether the surgeon can remove everything with clear margins. Risks include bleeding, bile leakage, infection and temporary liver weakness, and they are lower in high-volume centres. The liver regrows over weeks to months, and chemotherapy may be advised.

Expect around a week in hospital, followed by rest nearby before you fly. Avoid heavy lifting for about six weeks. Bring your scans, blood tests including liver function, and any earlier oncology records. Arrange surveillance imaging and tumour-marker tests with your home doctor, and avoid alcohol while the liver recovers.

Généralement adapté si

  • Liver cancer, bowel cancer metastases or a large benign liver tumour that can be removed with enough liver left.
  • Good liver function, with no cirrhosis or only well-compensated disease.
  • Fit for abdominal surgery after tumour board review.

Il peut falloir attendre si

  • Too little remaining liver may require preparation such as portal vein embolisation or another treatment.
  • Advanced cirrhosis with poor function makes resection risky.
  • Extensive spread in the liver or beyond may call for drug or ablation treatment instead.
Comment se préparer
  1. Bring CT or MRI scans, tumour markers and liver function tests; the surgeon measures the remaining liver volume.
  2. Tell your doctor about hepatitis, cirrhosis, alcohol use and earlier chemotherapy.
  3. Ask your doctor about adjusting blood thinners, diabetes and herbal medicines before surgery.
  4. Stop alcohol and smoking and follow nutrition advice to help liver regeneration.
  5. Plan a stay near the hospital for a week or two after discharge for blood tests.
Questions à poser à votre chirurgien
  1. What amount of liver will be removed and what will remain?
  2. Is a laparoscopic approach suitable for my tumour?
  3. How do you limit bleeding and when do patients need transfusion?
  4. What are your rates of bile leak and liver failure, and how do you treat them?
  5. Will I need chemotherapy or ablation as well?
  6. How often will I need scans and blood tests, and where?
Signaux d'alerte à surveiller en comparant les hôpitaux
  • Surgeon does not measure the future liver remnant before major resection.
  • No intensive care or high-dependency unit available for major liver surgery.
  • No tumour board or imaging review before surgery.
  • Surgeon cannot say how many liver resections are done yearly.
