About Watchman / LAA Closure

Under general anaesthesia, a catheter is passed up a vein from the groin and across the wall between the heart's upper chambers. Guided by transoesophageal echo, the device is released in the appendage, where most clots form in atrial fibrillation. It is a one-off implant, usually followed by a short course of blood thinners or antiplatelet tablets.

The device seals off the main source of clots, lowering stroke risk in selected patients, though it does not treat the irregular rhythm itself. Results depend on a good seal, your stroke risk and the operator's experience. Uncommon complications include fluid around the heart, device-related clots and leaks beside the device.

Most people stay one or two nights and resume normal activity within about two weeks. Bring ECG reports, echo findings and your blood-thinner history. You will need follow-up imaging, usually after about 45 days to a few months, so arrange that at home and follow the medicine plan exactly.

Usually a good fit if

  • Non-valvular atrial fibrillation with a high stroke risk, usually based on the CHA2DS2-VASc score.
  • History of serious bleeding or a bleeding risk that makes long-term anticoagulation unsuitable.
  • Patients who can take a short course of blood thinner or antiplatelet drugs after the implant.

May need to wait if

  • Clot already present in the left atrial appendage.
  • Unsuitable appendage shape or size, or a heart structure that prevents safe access.
  • Inability to take any antithrombotic medicine for even a short period after implant.
How to prepare
  1. Bring ECGs, your AF history, any stroke or bleeding history and your current blood thinner list.
  2. Expect blood tests and a transoesophageal echo or CT scan to size the appendage and rule out clots.
  3. Ask your cardiologist what to do with your blood thinner before and after; follow their instruction only.
  4. Tell the team about allergies, especially to nickel or contrast dye.
  5. Plan a short stay near the hospital for the early check and device follow-up.
Questions to ask your surgeon
  1. Why am I a better candidate for LAA closure than for continuing blood thinners?
  2. Which device do you use, and how will its size be selected?
  3. How many appendage closures have you performed, and what are the complication risks?
  4. What medicines will I take afterwards, and when will they be stopped?
  5. When will the device be checked for leaks or clot, and by what scan?
  6. Can I continue my AF rhythm treatment?
Red flags when comparing hospitals
  • Device offered with no stroke-risk or bleeding-risk assessment.
  • No transoesophageal echo or CT to size the appendage.
  • No imaging follow-up scan planned after implant.
  • Presented as a cure for atrial fibrillation itself.

Questions about Watchman / LAA Closure abroad

Which country is cheapest for Watchman / LAA Closure?

The lowest typical hospital prices are in India ($16,935), Thailand ($21,268), Turkey ($22,848), out of 9 countries we compare.

How long will I stay in hospital for Watchman / LAA Closure?

Most patients spend about 2 nights in hospital. Full recovery takes about 14 days; your surgeon will tell you when you are fit to fly home.

Does it cure atrial fibrillation?

No. The device lowers stroke risk by sealing the appendage, but the irregular rhythm remains, so rhythm and rate treatment may still be needed.

Will I stop blood thinners?

Often after a short period, but this depends on your follow-up scan. Your cardiologist will decide when and how.

Is it a major operation?

It is catheter-based with a small groin puncture, but a rare complication such as fluid around the heart can be serious.
