About Mitral Valve Replacement (Tissue)
Under general anaesthesia and with a heart-lung machine, the surgeon opens the chest, removes the damaged valve and stitches in a bioprosthetic valve, usually made from pig or cow tissue on a supporting frame. Blood thinners are generally needed for only the first few months, which is a major advantage. Some failed tissue valves can later be treated through a catheter.
Tissue valves restore blood flow and relieve symptoms, but they gradually wear, and may need replacing after about 10 to 15 years, sooner in younger patients. Outcomes depend on your age, heart function, other conditions and the surgeon's experience. Regular echocardiograms track how the valve is ageing.
Plan to stay near the hospital for about eight to ten days and allow roughly two months before heavy lifting. Bring your echocardiogram and angiogram reports, and arrange a cardiologist at home for check-ups. Carry the valve card, tell your dentist, and ask about antibiotic cover before dental work.
Usually a good fit if
- Severe mitral valve disease that cannot be repaired.
- Older patients, typically over 65, or younger patients unable or unwilling to take warfarin for life.
- Preference to avoid lifelong anticoagulation.
- Fitness for open-heart surgery.
May need to wait if
- Young age, where the tissue valve may wear out quickly and need repeat surgery.
- Valve disease that can be repaired instead.
- Severe kidney failure or other conditions that speed up tissue valve wear.
- High surgical risk better treated by a catheter-based option.
How to prepare
- Bring echocardiograms and heart reports; have blood tests, coronary assessment, dental review, and kidney and lung checks.
- Review blood thinners, heart and diabetes medicines with your doctor, who will tell you what to adjust.
- Treat dental infections before surgery.
- Discuss with the surgeon how long the tissue valve is expected to last at your age.
- Plan a long stay, rehabilitation, and short-term INR checks at home.
Questions to ask your surgeon
- Is repair possible, or is a tissue valve my best option?
- How long is this valve expected to last at my age?
- What blood thinner will I take, and for how long?
- If it wears out, can a catheter-based valve-in-valve procedure be used?
- How many mitral replacements does your team perform?
- What dental and infection precautions do I need?
Red flags when comparing hospitals
- Tissue valve recommended for a young patient without explaining the need for later reoperation.
- No echo-guided valve testing in the operating theatre.
- No cardiac intensive care.
- No plan for short-term warfarin or INR monitoring if used.
- Surgeon cannot name the valve type.
