About Total Mesorectal Excision (TME)

Under general anaesthesia the surgeon works along the natural plane around the rectum, called the mesorectal fascia, taking care to protect the nerves for bowel, bladder and sexual function. The approach may be open, laparoscopic or robotic. The bowel is rejoined, often with a temporary stoma, or a permanent stoma is made when the tumour is very low.

A clean, intact specimen is the main marker of a good operation and is closely linked to a lower risk of the cancer coming back. Results depend on tumour stage, whether radiotherapy or chemotherapy was given first, and surgeon experience. Bowel function can be altered for months, and some people have lasting changes.

Allow about a week in hospital and roughly six weeks before normal activity. If you have a stoma, a stoma nurse should teach you its care before you leave. Bring MRI, colonoscopy and pathology reports, and arrange follow-up with colonoscopy and CEA blood tests at home.

Usually a good fit if

  • Biopsy-proven rectal cancer without distant spread that cannot be treated by local excision alone.
  • Locally advanced disease after chemoradiotherapy, once restaged.
  • Fitness enough for major pelvic surgery.
  • A tumour distance from the anus that your surgeon judges allows sphincter preservation, or a stoma is accepted.

May need to wait if

  • Metastatic disease that has not been assessed by a tumour board.
  • Heart or lung disease that needs optimisation before general anaesthesia.
  • Malnutrition, severe anaemia or active infection needing correction first.
  • Very early cancers where local excision might be enough, if suitable.
How to prepare
  1. Bring colonoscopy and biopsy reports, pelvic MRI and CT of the chest and abdomen so the team can stage the tumour.
  2. Ask whether radiotherapy or chemoradiotherapy is planned before surgery and allow time for it to take effect.
  3. Discuss bowel preparation and any antibiotics your surgeon wants; do not alter medicines yourself.
  4. Meet a stoma nurse for site marking and teaching before surgery.
  5. Arrange to stay nearby for several weeks, as stoma care and wound checks are needed.
Questions to ask your surgeon
  1. Is chemoradiotherapy advised before surgery in my case?
  2. Will I need a temporary or permanent stoma, and for how long?
  3. Is the operation open, laparoscopic or robotic, and what is your experience with it?
  4. How will the mesorectum quality and margins be reported?
  5. What bowel, bladder and sexual function changes can I expect?
  6. How will follow-up scans and CEA tests be arranged back home?
Red flags when comparing hospitals
  • No pelvic MRI before surgery.
  • No mention of a stoma or of bowel and sexual function risks.
  • A surgeon who performs few rectal cancer operations each year.
  • No pathology report commitment on mesorectal quality and margins.

Questions about Total Mesorectal Excision (TME) abroad

Which country is cheapest for Total Mesorectal Excision (TME)?

The lowest typical hospital prices are in India ($14,695), Thailand ($18,920), Turkey ($21,598), out of 9 countries we compare.

How long will I stay in hospital for Total Mesorectal Excision (TME)?

Will I need a stoma?

Many people have a temporary ileostomy to protect the bowel join, often reversed after a few months. A permanent stoma is needed when the tumour is very low.

Will my bowel function return to normal?

Frequent stools, urgency and clustering are common after low joins, a pattern called low anterior resection syndrome. It improves over months with diet and physiotherapy.

Are sexual and urinary problems possible?

Pelvic nerve injury can affect them, which is why nerve-sparing technique matters. Discuss risks in advance.

Is radiotherapy always required?

Not always. Pelvic MRI staging guides whether it is used before surgery.
