About Total Gastrectomy

Under general anaesthesia the surgeon removes the stomach together with surrounding lymph nodes and the omentum, then builds a new route for food, usually a Roux-en-Y reconnection. It can be done through open surgery or by keyhole methods in experienced centres. Chemotherapy before or after surgery is often part of the overall plan.

The operation offers the best chance of removing the cancer completely when it is found before spreading widely. Outcomes depend on stage, the completeness of the lymph node clearance and the surgeon's volume. Living without a stomach means smaller meals, vitamin B12 injections for life and a risk of weight loss that needs dietitian support.

Plan around eight days in hospital and about two months before you feel back to normal activity. You will need to stay near the hospital until feeding is established. Bring pathology and scan reports, and arrange a dietitian and regular blood tests at home, particularly for vitamin B12 and iron.

Usually a good fit if

  • Stomach cancer involving the upper or body of the stomach, or diffuse-type cancer where the whole organ is at risk.
  • Disease judged removable by a multidisciplinary team after staging.
  • Good enough fitness and nutrition to tolerate major abdominal surgery.
  • Selected hereditary conditions with a high risk of stomach cancer, after specialist counselling.

May need to wait if

  • Cancer that has spread widely to the liver or peritoneum, where surgery may not help.
  • Severe heart or lung disease making major surgery unsafe.
  • Severe malnutrition not yet corrected.
  • Uncontrolled bleeding or sepsis that needs stabilising first.
How to prepare
  1. Bring endoscopy and biopsy reports, staging CT and any PET scan so the team can confirm the extent of disease.
  2. Ask about nutritional assessment; weight loss and anaemia are often corrected before surgery.
  3. Review medicines such as blood thinners and diabetes drugs with your doctor; never stop them yourself.
  4. Stop smoking and begin gentle breathing exercises as advised to lower chest complications.
  5. Plan to remain near the hospital for at least two weeks and have a carer travel with you.
Questions to ask your surgeon
  1. Is a total gastrectomy needed, or could a subtotal removal work for my tumour?
  2. Will I have chemotherapy before or after surgery, and where?
  3. How wide is the lymph node removal, and why?
  4. How often do leaks from the join occur at your centre, and how are they managed?
  5. How will my eating, vitamin B12 and iron be managed afterwards?
  6. Who can I contact from home about eating problems or weight loss?
Red flags when comparing hospitals
  • No staging scans or multidisciplinary tumour board before recommending surgery.
  • No dietitian or plan for vitamin B12 and nutrition after the stomach is removed.
  • A surgeon who rarely performs oesophagogastric surgery.
  • No intensive care or interventional radiology backup in the hospital.

Questions about Total Gastrectomy abroad

Which country is cheapest for Total Gastrectomy?

The lowest typical hospital prices are in India ($16,263), Thailand ($20,553), Turkey ($23,668), out of 9 countries we compare.

How long will I stay in hospital for Total Gastrectomy?

Most patients spend about 8 nights in hospital. Full recovery takes about 56 days; your surgeon will tell you when you are fit to fly home.

How will I eat without a stomach?

You eat small meals six or more times a day. The oesophagus joins the intestine, which adapts over months. A dietitian guides the progress.

Will I need vitamin injections?

Yes. Without the stomach you cannot absorb vitamin B12, so injections are needed for life, plus iron and calcium if advised.

Will I lose weight?

Most people lose weight in the first months. The aim is to limit it with calorie-dense food and supplements.

Is chemotherapy always needed?

Often yes, before or after surgery, depending on stage. Your oncologist will confirm.
