About Temporal Lobectomy (Epilepsy)

Evaluation first confirms that seizures arise from one temporal lobe, using video-EEG, MRI and memory and language testing. The surgeon then removes the front part of the lobe and the hippocampus and amygdala through an opening in the skull. Smaller, targeted removals are sometimes used to protect memory and language.

For well-selected patients, especially those with hippocampal scarring on MRI, many become free of disabling seizures. Results depend on how clearly the seizure focus is defined and the side operated on, since left-sided surgery carries a greater risk to verbal memory. Anti-seizure medicines are usually continued for a time afterwards.

Plan about six days in hospital and a recovery of several weeks. Bring your full EEG, MRI and neuropsychology reports and a seizure diary. Arrange epilepsy follow-up at home, as medicine reduction and any driving decisions are made slowly, and have a companion with you for early weeks.

Usually a good fit if

  • Drug-resistant temporal lobe epilepsy, usually after two suitable medicines failed.
  • MRI showing hippocampal sclerosis or a temporal lesion matching EEG findings.
  • Seizures that limit work, safety or quality of life.
  • Patients with acceptable memory and language risk on testing.

May need to wait if

  • Seizures from both temporal lobes independently.
  • High risk to memory, especially if the other side is weak.
  • Non-epileptic seizures.
  • Severe illness or active infection.
How to prepare
  1. Bring EEG, MRI and PET reports and a list of anti-epileptic medicines tried.
  2. Do not change anti-epileptic doses unless your neurologist instructs you, as the team will plan medicine handling.
  3. Expect video-EEG, neuropsychological testing and sometimes Wada or functional MRI language testing.
  4. Keep a seizure diary and video of typical seizures.
  5. Plan a longer stay for pre-surgical evaluation.
Questions to ask your surgeon
  1. What are my chances of becoming seizure-free with this operation?
  2. Will you remove the whole anterior temporal lobe or a selective part?
  3. What is the risk to memory and word-finding?
  4. Will language be mapped while I am awake?
  5. How many temporal lobectomies do you do each year?
  6. How long should I continue my medicines?
Red flags when comparing hospitals
  • No video-EEG or neuropsychology service.
  • Surgery without MRI or EEG evidence of temporal origin.
  • Guarantee of cure.
  • No plan for medicine management and follow-up.
