About Endoscopic Skull Base Surgery
A neurosurgeon and an ear, nose and throat surgeon often work together. Thin endoscopes and instruments pass through the nostrils and sinuses, and a small opening is made in the bone at the skull base. The lesion is removed, and the opening is repaired with tissue grafts or a flap to prevent spinal fluid leaking.
Because there is no external cut, recovery is usually gentler than with open skull base surgery. How completely a lesion can be removed depends on its type, size and relation to nerves and vessels. The most important risks are spinal fluid leak and infection, and results depend heavily on the team's experience with these operations.
Plan for about five days in hospital and several weeks of avoiding nose-blowing, straining and heavy lifting. Bring your MRI and CT images and pathology reports. Arrange follow-up with an ENT and neurosurgical team at home, and know the signs of a leak, such as clear fluid from the nose or severe headache.
Usually a good fit if
- Pituitary or other tumours at the skull base reachable through the nose.
- Cerebrospinal fluid leaks or encephaloceles needing repair.
- Selected meningiomas, craniopharyngiomas, chordomas or clival lesions.
- Patients fit for general anaesthesia.
May need to wait if
- Tumours extending widely beyond what can be safely reached through the nose.
- Active sinus infection or uncorrected clotting problems.
- Severe medical illness that makes anaesthesia unsafe.
- Lesions better treated by radiosurgery or open surgery.
How to prepare
- Bring MRI and CT images on disc, plus any hormone test results if a pituitary or related lesion is involved.
- Review blood thinners and nasal medicines with your doctor, who will instruct any changes.
- Expect blood tests, hormone checks and a vision test if the tumour is near the optic nerves.
- Treat any sinus infection or dental infection first, as advised.
- Tell the team about allergies, and plan to stay for monitoring for spinal fluid leaks.
Questions to ask your surgeon
- Is the endoscopic route right for my lesion, and what might need open surgery?
- How many endoscopic skull base operations does your team do each year?
- How will you repair the skull base to prevent spinal fluid leaks?
- What risks apply to my vision and hormones?
- Will I need hormone replacement or radiotherapy afterwards?
- What nose care and follow-up scans do I need at home?
Red flags when comparing hospitals
- Surgery by a single surgeon without a combined skull-base team.
- No image guidance.
- No plan for skull-base reconstruction or leak management.
- No hormone and eye assessment before and after.
Questions about Endoscopic Skull Base Surgery abroad
Which country is cheapest for Endoscopic Skull Base Surgery?
The lowest typical hospital prices are in India ($21,640), Thailand ($26,680), Turkey ($30,908), out of 9 countries we compare.
How long will I stay in hospital for Endoscopic Skull Base Surgery?
Most patients spend about 5 nights in hospital. Full recovery takes about 35 days; your surgeon will tell you when you are fit to fly home.
Is there a scar?
No external incision is needed. The surgery is done through the nose, though a small graft site, such as the thigh, may be used for repair.
Could I get a spinal fluid leak?
This is a recognised risk, so the skull base is reconstructed carefully. Report clear watery drainage promptly.
Will it affect smell?
Smell can be reduced temporarily or occasionally permanently, depending on the approach. Ask your surgeon.
When can I fly home?
Commonly after one to two weeks once there is no leak. Your surgeon will confirm.
