About Flow Diverter Placement (Pipeline)

A catheter is guided from the groin or wrist to the brain, and the flow diverter is released across the aneurysm neck within the parent artery. Unlike coiling, the aneurysm sac is not necessarily filled. Blood clots inside it over time while the artery wall heals along the mesh. Antiplatelet medicines are essential to keep the stent open.

Closure happens gradually over months, and follow-up scans confirm when it is complete. Success depends on the aneurysm's size, location and shape, and on taking antiplatelet tablets exactly as directed. Risks include clot formation, stroke and, rarely, rupture of a large aneurysm after treatment, so careful selection is important.

Plan around four days in hospital and several weeks of reduced activity. Antiplatelet medicines usually continue for months, so arrange who will monitor them at home. Carry your device details and angiogram images, and book follow-up angiograms or MRA at the intervals your team advises.

Usually a good fit if

  • Large or wide-necked aneurysms not suited to coiling or clipping.
  • Aneurysms on the internal carotid artery, such as the paraophthalmic segment.
  • Fusiform or dissecting aneurysms.
  • Patients who can safely take antiplatelet medicine.

May need to wait if

  • Inability to take dual antiplatelet therapy.
  • A recently ruptured aneurysm, where flow diversion carries more risk of bleeding.
  • Aneurysms involving key branch vessels that may be blocked by the mesh.
  • Allergy to contrast not prepared for.
How to prepare
  1. Bring CT angiogram or catheter angiogram images on disc and details of any previous treatment.
  2. Start and continue dual antiplatelet medicines exactly as your doctor instructs, often for days before the procedure, with a platelet test.
  3. Review all other medicines, and tell the team about allergies and kidney problems.
  4. Expect blood tests and a platelet function check.
  5. Plan how you will obtain and take antiplatelet tablets for months, including when travelling.
Questions to ask your surgeon
  1. Why is a flow diverter better than coiling or clipping for my aneurysm?
  2. How long will the aneurysm take to close, and how will we check?
  3. Which antiplatelet medicines, and for how long?
  4. What are the risks of blocked branch vessels or bleeding?
  5. How many flow diverters do you place each year?
  6. What follow-up angiography is needed?
Red flags when comparing hospitals
  • No antiplatelet or platelet testing plan.
  • No neurosurgical back-up if complications occur.
  • Claims that the aneurysm closes immediately.
  • Operator unable to share their flow-diverter experience.
