About Surgical Aortic Valve Replacement (SAVR)

Under general anaesthesia and with a heart-lung machine, the surgeon opens the breastbone, removes the diseased valve and sews in the new one. Mechanical valves last for decades but need lifelong warfarin. Tissue valves usually avoid long-term blood thinners but wear out sooner. Some centres use smaller incisions, and bypass grafts can be added in the same operation.

Replacing a severely diseased valve relieves breathlessness, chest pain and blackouts, and improves survival. Outcomes depend on your age, the state of the heart muscle, other conditions and the valve chosen. Mechanical valves need regular blood-test monitoring, while tissue valves may need replacing later, sometimes by a catheter procedure.

Plan for about a week in hospital and around two months for the breastbone to heal, with no heavy lifting during that time. Bring scans and previous reports. If you receive a mechanical valve, make sure you can arrange INR blood testing at home, and expect antibiotic cover before dental work.

Usually a good fit if

  • Severe aortic stenosis with symptoms such as breathlessness, chest pain or fainting.
  • Severe aortic regurgitation causing symptoms or heart enlargement.
  • Younger or lower-risk patients for whom a durable surgical valve is preferred over a catheter valve.

May need to wait if

  • Active infection of the heart valve or elsewhere, which must be treated first.
  • Very frail or high-risk patients, for whom a catheter valve (TAVI) may be safer.
  • Severe untreated lung, liver or kidney disease that needs optimisation before surgery.
How to prepare
  1. Bring your echo, angiogram or CT, and a full list of medicines; expect repeat heart scans and blood tests.
  2. Have a dental check and treat any infection before surgery, since oral bacteria can infect a new valve.
  3. Ask your cardiologist whether to continue blood thinners, blood pressure tablets or diabetes medicines; follow their instruction only.
  4. Stop smoking and practise breathing exercises, because they lower lung complications.
  5. Plan your flight home with the surgeon, usually two to four weeks after surgery.
Questions to ask your surgeon
  1. Should I have a mechanical or a tissue valve, and why at my age?
  2. Is TAVI an option for me instead of open surgery?
  3. How many aortic valve replacements do you perform annually, and what are your results?
  4. Will you need to bypass any coronary arteries at the same time?
  5. Which brand of valve will you use, and how will I get a valve card?
  6. What long-term medicines and monitoring will I need?
Red flags when comparing hospitals
  • Mechanical valve advised without a clear explanation of lifelong warfarin and monitoring.
  • No heart team discussion comparing surgery with TAVI.
  • Surgeon or unit that cannot state its yearly valve volume.
  • No plan for INR monitoring or valve follow-up back home.
