About Spinal Tumour Resection
The surgeon removes part of the vertebral bone to expose the tumour and uses a microscope and nerve monitoring to protect the spinal cord. Tumours inside or beside the cord, such as meningiomas and schwannomas, can often be removed completely. If the spine is weakened, metal screws and rods may be used to stabilise it.
Results depend on the tumour type and position, and how long it has been pressing. Benign tumours may be cured by complete removal. Pain usually improves first, while weakness and numbness recover more slowly and sometimes only partly. Further treatment, such as radiotherapy, may follow for malignant tumours.
Plan around a week in hospital and a recovery of several weeks, with physiotherapy as an important part. Bring your MRI images and any prior reports. Arrange rehabilitation and oncology or neurosurgical follow-up at home, and have help available because walking and bladder function may take time to recover.
Usually a good fit if
- Spinal tumour causing pain, weakness, numbness or bladder or bowel problems.
- Benign tumours such as meningioma, schwannoma or ependymoma where full removal is possible.
- Metastatic tumour causing cord compression in a patient fit for surgery.
- Spinal instability from tumour needing stabilisation.
May need to wait if
- Widespread cancer with very limited life expectancy, where surgery may not help.
- Severe medical illness that makes surgery unsafe.
- Complete paralysis for a long period, where recovery is unlikely.
- Active infection or uncorrected clotting problems.
How to prepare
- Bring MRI of the whole spine on disc and any biopsy or pathology reports.
- Review steroids, blood thinners and pain medicines with your doctor, who will advise any change.
- Expect blood tests, anaesthetic review and sometimes angiography or embolisation for vascular tumours.
- Note current strength, sensation and bladder or bowel function so changes can be compared.
- Plan for a physiotherapy-led recovery and an escort home.
Questions to ask your surgeon
- What type of tumour do you think this is, and can it be fully removed?
- Will you use neuromonitoring and spinal stabilisation?
- What is the risk to strength and sensation given where the tumour lies?
- How many spinal tumour operations does your team do each year?
- What further treatment, such as radiotherapy, may I need?
- What rehabilitation and scans are needed at home?
Red flags when comparing hospitals
- No intra-operative neuromonitoring.
- No spinal MRI of the whole spine before operating.
- No access to pathology and oncology follow-on.
- Promise that nerve function will recover completely.
