About Spinal Cord Stimulator (SCS)
A trial comes first, with temporary leads placed through a needle into the space around the spinal cord and connected to an external generator for about one to two weeks. If pain falls enough, permanent leads and a small battery are implanted under the skin, usually with sedation. Different waveforms exist, including tingling-free ones.
The device is meant to reduce pain, often by about half, rather than remove it entirely, and many people use fewer painkillers. Success depends on the cause of pain, careful patient selection and programming. It tends to help nerve and persistent post-surgical back and leg pain more than other types of pain.
Plan a couple of days in hospital for the implant, in addition to the trial period beforehand. Avoid bending, twisting and heavy lifting for several weeks while the leads settle. Make sure a pain clinic at home can reprogram and recharge-check the device, and carry your implant card for MRI and airport security.
Usually a good fit if
- Chronic neuropathic leg or back pain, such as failed back surgery syndrome.
- Complex regional pain syndrome or painful diabetic neuropathy that remains after standard care.
- A successful trial with clear pain reduction.
- Patients with realistic goals who can use the device.
May need to wait if
- Pain without a nerve cause, or untreated psychological problems.
- Active infection or a bleeding disorder.
- Need for frequent MRI that the device does not allow.
- Failed trial.
How to prepare
- Bring pain records, MRI scans and a list of treatments and medicines tried.
- Do not alter pain, anticoagulant or antiplatelet medicines unless your doctor instructs you, as these affect epidural procedures.
- Expect a psychological assessment, which is standard before stimulator implants.
- Treat any skin or urinary infection first.
- Plan for a trial period and several days of limited activity, and note travel restrictions after implant.
Questions to ask your surgeon
- Is my pain type likely to respond to stimulation?
- What trial will I have, and what counts as success?
- Which device, rechargeable or not, and is it MRI conditional?
- How many implants do you do each year?
- What are the risks of lead movement, infection and device failure?
- Who programmes the device when I am home?
Red flags when comparing hospitals
- Permanent implant without a trial period.
- No psychological assessment.
- Promise of complete pain relief.
- No plan for remote programming or follow-up.
