About Radical Prostatectomy (Robotic)

The surgeon operates robotic arms from a console, using a magnified 3D view to remove the prostate and seminal vesicles, and sometimes pelvic lymph nodes. The bladder is then joined to the urethra over a catheter. When the cancer allows, the delicate nerves beside the prostate are preserved to protect erectile function. Operating time is typically two to four hours.

Results depend on cancer grade and stage, PSA, surgical margins and the surgeon's experience. Leakage of urine is common early and usually improves over several months with pelvic floor exercises. Erection recovery varies with age, pre-operative function and nerve sparing. A rising PSA after surgery may signal recurrence and prompt further treatment.

Plan for a one to three night hospital stay and a catheter for about a week or two, so remain near the hospital until it is removed. Avoid lifting for around six weeks. Bring MRI, biopsy and PSA results, and arrange PSA tests and pelvic floor physiotherapy with your urologist at home.

Usually a good fit if

  • Localised prostate cancer that appears confined to the gland, in men with a life expectancy of ten years or more.
  • Intermediate or high-risk cancer where a multidisciplinary team recommends surgery, sometimes with later radiation.
  • Fit for general anaesthesia and willing to accept the risks to continence and erections.

May need to wait if

  • Cancer that has spread to bones or distant organs is usually treated with hormone or drug therapy instead.
  • Severe heart or lung disease may make anaesthesia and robotic positioning unsafe.
  • Very low-risk cancers may be better managed by active surveillance, which your doctor can discuss.
How to prepare
  1. Bring biopsy results, PSA, prostate MRI and any bone or PSMA scans so staging and nerve sparing can be planned.
  2. Ask your doctor about adjusting blood thinners, diabetes and blood pressure medicines before surgery.
  3. Start pelvic floor exercises before surgery, taught by a physiotherapist, to help urinary control.
  4. Discuss erectile function, fertility and sperm storage before the operation.
  5. Plan to remain near the hospital until the catheter is removed (usually 7–14 days after surgery) and the team clears you to fly.
Questions to ask your surgeon
  1. Is surgery, radiation or active surveillance best for my risk group?
  2. How many robotic prostatectomies do you personally perform each year?
  3. Can the nerves be spared on one or both sides, and what are realistic chances of erections?
  4. How long until urinary control returns, and what help is available with pelvic floor rehabilitation?
  5. What do pathology margins and lymph nodes mean for further treatment?
  6. How will PSA be followed and who monitors it at home?
Red flags when comparing hospitals
  • Surgeon cannot say how many robotic prostatectomies they have done.
  • No prostate MRI or staging is reviewed before surgery.
  • Nerve sparing is promised without considering your cancer risk.
  • Surgery is advised for every patient without discussing radiation or surveillance.
