About Radical Cystectomy

The surgeon removes the bladder, the lymph nodes in the pelvis, and the prostate and seminal vesicles in men, or the uterus, ovaries and part of the vagina in women. Urine is then diverted, either through an ileal conduit to a stoma bag, or a continent reservoir, or a new bladder made from bowel connected to the urethra. It can be open or robotic.

Cancer outcomes depend on the stage, lymph node involvement and whether chemotherapy is given before surgery. It is a major operation with notable complications, such as infection, bowel problems and changes in sexual function, so hospital volume and surgical experience are important. The type of urinary diversion affects daily life, and you should discuss it fully.

Plan for about a week to ten days in hospital and stoma or catheter teaching before discharge. Stay nearby for a short time afterwards to check wounds and kidney function. Bring cystoscopy, imaging and pathology reports, and arrange stoma nurse support, supplies and surveillance scans with your doctor at home.

Usually a good fit if

  • Muscle-invasive bladder cancer, or high-risk non-invasive cancer not controlled by bladder-sparing treatment.
  • Kidney function adequate for the chosen urinary diversion.
  • Fit for a major operation, with a multidisciplinary team agreeing on surgery.

May need to wait if

  • Spread to distant organs usually means drug treatment is the main option.
  • Poor kidney, heart or lung function may restrict surgery or diversion choice.
  • A neobladder is generally unsuitable with poor kidney function, bowel disease or inability to manage self-catheterisation.
How to prepare
  1. Bring cystoscopy, biopsy, CT urogram and chest imaging results so staging is complete.
  2. Meet the stoma nurse before surgery to mark the stoma site and learn how diversions work.
  3. Ask your doctor about chemotherapy before surgery, kidney function tests and adjusting blood thinners.
  4. Stop smoking, and optimise diabetes and nutrition before the operation.
  5. Plan a longer stay near the hospital, with a companion trained in stoma care.
Questions to ask your surgeon
  1. Which urinary diversion is suitable for me, an ileal conduit or a neobladder?
  2. Is pre-operative chemotherapy recommended?
  3. Is the surgery open or robotic, and how many do you perform each year?
  4. How will sexual and bowel function be affected?
  5. What are the main early and late complications and how are they managed?
  6. Who can help me with stoma or catheter problems at home?
Red flags when comparing hospitals
  • Surgeon cannot say how many cystectomies they perform annually.
  • No stoma nurse or counselling before surgery.
  • No discussion of chemotherapy before surgery, when it is standard.
  • No staging scans before surgery.

Which country is cheapest for Radical Cystectomy?

The lowest typical hospital prices are in India ($19,083), Thailand ($22,698), Turkey ($25,750), out of 9 countries we compare.

How long will I stay in hospital for Radical Cystectomy?

Most patients spend about 9 nights in hospital. Full recovery takes about 70 days; your surgeon will tell you when you are fit to fly home.

What is an ileal conduit?

A piece of small bowel used as a channel so urine drains from the kidneys through a stoma on the abdomen into a bag.

What is a neobladder?

A reservoir made from bowel, joined to the urethra, so you pass urine by relaxing and using abdominal muscles. Some people need to self-catheterise.

Will sexual function change?

In men, erections can be affected and ejaculation is lost; in women, vaginal changes can occur. Nerve-sparing techniques and later treatments can help.

How long is recovery?

Hospital stay is around nine days, but returning to full strength often takes two to three months or longer.
