About Micro-TESE
Under general anaesthesia, the surgeon opens the scrotum and makes a single incision in the testicle. Through a high-power microscope, tubules that look larger and fuller, which are the most likely to contain sperm, are selected and removed in tiny pieces while healthy tissue is spared. The embryologist examines the tissue in the next room, and any sperm found are frozen or used with ICSI.
Sperm are found in roughly half of men with non-obstructive azoospermia, but this varies widely with the underlying cause, such as Klinefelter syndrome or an earlier undescended testicle. Finding sperm does not guarantee a pregnancy, since the IVF cycle still depends on the woman's age and egg quality. Your doctor can discuss your chances after hormone and genetic tests.
You usually go home the same day or after one night. Expect swelling and aching for a few days, with a short course of pain relief and avoiding heavy exertion for about a week. A blood test for hormones is worth repeating, as testosterone can fall temporarily. Coordinate timing with the IVF clinic and bring your hormone and genetic reports.
Usually a good fit if
- Non-obstructive azoospermia with no sperm in the semen.
- Failed conventional TESE or earlier biopsy that found no sperm.
- Klinefelter syndrome or other genetic causes, when advised by a fertility specialist.
- Men whose partners are ready for ICSI treatment.
May need to wait if
- Obstructive azoospermia, where simpler retrieval such as MESA or PESA may be adequate.
- Active testicular or scrotal infection, which should be treated first.
- Unwillingness to accept that sperm may not be found, since success varies by cause.
- Medical conditions that make general anaesthesia unsafe until optimised.
How to prepare
- Hormone tests (FSH, LH, testosterone) and a genetic assessment, including karyotype and Y-chromosome microdeletion testing, are normally done first.
- Review medicines, including any hormone therapy, with your doctor and change nothing yourself.
- Fast before general anaesthesia as instructed, and arrange for someone to accompany you home.
- Coordinate the surgery date with the partner's IVF cycle or sperm freezing plan.
- Bring previous semen analyses, hormone reports and any earlier biopsy results.
Questions to ask your surgeon
- What is the likely cause of my azoospermia, and what are my chances of finding sperm?
- Will an embryologist check the samples during the operation?
- How do you protect testosterone production, and will you check hormone levels afterwards?
- Will sperm be used fresh or frozen, and for how long can it be stored?
- Do we need genetic counselling before treatment?
- What follow-up is needed at home?
Red flags when comparing hospitals
- Promising guaranteed sperm retrieval.
- No operating microscope or on-site embryology laboratory.
- No genetic testing or counselling before surgery.
- No plan to monitor testosterone afterwards.
- Surgeon with little experience of microsurgical sperm retrieval.
Questions about Micro-TESE abroad
Which country is cheapest for Micro-TESE?
The lowest typical hospital prices are in India ($5,550), Thailand ($6,540), Turkey ($7,850), out of 9 countries we compare.
What are the chances of finding sperm?
This depends on the cause. Retrieval rates vary widely between men, so ask your specialist for an estimate based on your hormone results and genetic findings.
Does micro-TESE lower testosterone?
Some men have a temporary drop, and rarely a longer-term one. The microscope helps by removing less tissue. Hormone levels are rechecked after surgery.
Can the sperm be frozen?
Yes, often. Freezing allows several ICSI attempts from a single operation.
Will I need to be in hospital overnight?
Usually not. Most men go home the same day.
