About MESA (Microsurgical Epididymal Sperm Aspiration)

Under local anaesthesia with sedation, or occasionally general anaesthesia, the surgeon makes a small cut in the scrotum and uses an operating microscope to find the epididymis. A tiny opening is made in a tube and fluid containing sperm is drawn out. The sperm are checked straight away and usually frozen, then used later with ICSI as part of IVF.

Sperm are found in most men with an obstructive cause, such as previous vasectomy or congenital absence of the vas deferens, and the sperm collected are generally of good quality. What follows depends on the IVF cycle: the age of the woman and the quality of eggs and embryos matter most. Sperm retrieval alone does not create a pregnancy.

The procedure is a day case, and you can typically walk the same day, with scrotal soreness and swelling for a few days. Avoid heavy lifting and strenuous exercise for about a week. If frozen sperm are used, check that your IVF clinic and the sperm laboratory coordinate timing, and bring your semen reports and hormone results.

Usually a good fit if

  • Obstructive azoospermia, where sperm production is normal but there is a blockage.
  • Congenital bilateral absence of the vas deferens.
  • Failed vasectomy reversal, or blockage after infection or surgery.
  • Men who wish to use the retrieved sperm for ICSI, as MESA yields high numbers of sperm.

May need to wait if

  • Non-obstructive azoospermia, where sperm production is impaired; micro-TESE is more likely to be needed.
  • Untreated genital infection or skin infection of the scrotum.
  • Bleeding disorders or use of blood thinners that your doctor has not yet reviewed.
  • Partner not ready for IVF-ICSI treatment, since retrieved sperm is normally used for ICSI.
How to prepare
  1. Semen analysis showing absence of sperm, hormone tests (FSH, testosterone) and a scrotal ultrasound.
  2. Genetic tests such as cystic fibrosis gene screening if the vas deferens is absent, and counselling for both partners.
  3. Review blood-thinning medicines and supplements with your doctor.
  4. Plan with the clinic whether sperm will be used fresh or frozen, and coordinate with the partner's IVF timing.
  5. Wear supportive underwear and arrange for someone to accompany you home.
Questions to ask your surgeon
  1. Is my azoospermia obstructive, and how was that confirmed?
  2. Will the sperm be used fresh or frozen, and how many vials can be stored?
  3. Do you perform the procedure with an operating microscope?
  4. What happens if no sperm are found?
  5. Should we test for genetic causes such as cystic fibrosis gene variants?
  6. What follow-up is needed at home after the surgery?
Red flags when comparing hospitals
  • Diagnosis of azoospermia not clearly established before surgery.
  • No embryologist present to check sperm during the procedure.
  • No discussion of genetic testing in men with absent vas deferens.
  • No plan for sperm freezing.
  • Surgeon cannot explain the difference between MESA, PESA and TESE.

Questions about MESA (Microsurgical Epididymal Sperm Aspiration) abroad

Which country is cheapest for MESA (Microsurgical Epididymal Sperm Aspiration)?

The lowest typical hospital prices are in India ($4,108), Thailand ($4,755), Turkey ($5,663), out of 9 countries we compare.

Is MESA painful?

You will have anaesthesia during the operation. Afterwards, soreness, swelling and bruising of the scrotum are common for a few days and are controlled with simple painkillers.

Will I produce sperm normally afterwards?

MESA does not damage sperm production, but the epididymis is a tiny structure, so your surgeon may advise against repeated procedures.

Can the sperm be frozen?

Yes. Sperm retrieved by MESA is often frozen in several vials for future ICSI cycles.

What is the difference between MESA and TESE?

MESA takes sperm from the epididymis for blockage cases. TESE takes it from testicular tissue and is used when production itself is impaired.
