About J-Pouch (IPAA)
The operation is done under general anaesthesia and is often staged in two or three operations. The surgeon removes the colon and rectum, folds the end of the small bowel into a J shape to form a pouch and stitches it to the anal canal. A temporary ileostomy usually protects the join and is reversed after a couple of months.
Most people have good long-term function, with a typical number of around five to eight bowel movements a day, though results vary. Complications include pouchitis, an inflammation that usually responds to antibiotics, blockage, leakage and reduced fertility in women. Outcomes depend on the diagnosis, the surgeon's experience and careful selection, and a few people eventually need the pouch removed.
Plan for a long stay near the surgical team: roughly eight days in hospital and a number of weeks of recovery before travel, and a second trip for ileostomy reversal. Bring your colitis history, colonoscopy and pathology reports. Arrange a gastroenterologist and stoma nurse at home, and expect lifelong follow-up of the pouch.
Usually a good fit if
- Ulcerative colitis not controlled by medicine, or with dysplasia.
- Familial adenomatous polyposis.
- Patients wishing to avoid a permanent ileostomy.
- Good anal sphincter function and fitness for major surgery.
May need to wait if
- Crohn disease, since a pouch carries higher risk of failure.
- Weak sphincter or incontinence.
- Rectal cancer that is low and involves the sphincter.
- Severe malnutrition, or high-dose steroids that need optimisation.
How to prepare
- Colonoscopy with biopsies, plus blood tests and nutrition review.
- Meet the stoma nurse for marking and teaching before surgery.
- Review steroids, biologics and other medicines with your doctor; change nothing yourself.
- Discuss pouch function, fertility effects and the staged plan with your surgeon.
- Plan a longer stay and a second trip for ileostomy closure.
Questions to ask your surgeon
- Am I suitable for a pouch rather than a permanent ileostomy?
- Will surgery be in two or three stages?
- What are the risks of pouchitis, leaks and pouch failure?
- How many pouches do you perform each year?
- What will my bowel function be like?
- What follow-up and pouch surveillance are needed at home?
Red flags when comparing hospitals
- Surgeon rarely performs pouch surgery.
- No discussion of possible Crohn disease.
- No stoma nurse involvement.
- No staged plan explained.
- No long-term pouch surveillance plan.
