About Immunotherapy (PD-1 / PD-L1 course)
The drugs are antibodies, such as pembrolizumab or nivolumab, given as an infusion every few weeks, usually as an outpatient. Treatment may be used alone or with chemotherapy or other targeted drugs, and a test of PD-L1 or other biomarkers may guide the choice. Courses can last up to about two years, depending on the cancer and response.
Some people have deep and lasting responses, while others do not respond, so results depend on the cancer type, tumour biomarkers, previous treatment and general health. Because the immune system is activated, side effects can include inflammation of the skin, bowel, lungs, liver or hormone glands, which need prompt treatment. Scans every few months assess response.
Because infusions repeat, consider whether later doses can be given nearer home. If you travel, plan to remain close to the hospital for the first cycles and know the symptoms that need urgent attention. Bring biomarker reports and prior treatment records, and arrange regular blood and thyroid tests with your home oncologist.
Usually a good fit if
- A cancer type and stage for which PD-1 or PD-L1 drugs are approved or supported by evidence.
- Biomarker results, where relevant, that favour response.
- Able to attend scheduled infusions and report symptoms promptly.
May need to wait if
- Active severe autoimmune disease, or an organ transplant, may be worsened by immunotherapy and needs specialist review.
- Needing high-dose steroids or immunosuppression may reduce effectiveness or raise risk.
- Active serious infection or poor organ function may need treatment first.
How to prepare
- Bring pathology, biomarker results such as PD-L1 or MSI status, and scans so the team can confirm eligibility.
- Expect baseline thyroid, liver, kidney and cortisol tests and a chest scan before starting.
- Tell your doctor about autoimmune disease, transplants, steroids or any immune-suppressing drugs.
- Carry a card describing your treatment, as emergency teams must know about immunotherapy.
- Plan how you will contact the hospital if side effects begin after you return home.
Questions to ask your surgeon
- Which drug and schedule are recommended, and for how many cycles?
- Is my cancer type or biomarker result suitable for this treatment?
- Which immune side effects should I watch for and who do I call urgently?
- How will you judge response, and what happens if the scan looks worse at first?
- Can infusions be given nearer to home, and how are my records shared?
- What happens if I have to stop because of side effects?
Red flags when comparing hospitals
- Treatment is offered without biomarker or pathology confirmation where these are needed.
- No emergency plan or contact for immune side effects, such as severe diarrhoea or breathlessness.
- Promises of cure, or use outside accepted indications without a clear explanation.
- No baseline thyroid, liver and kidney tests before starting.
Questions about Immunotherapy (PD-1 / PD-L1 course) abroad
Which country is cheapest for Immunotherapy (PD-1 / PD-L1 course)?
The lowest typical hospital prices are in India ($16,035), Thailand ($18,395), Turkey ($22,015), out of 9 countries we compare.
How is immunotherapy different from chemotherapy?
It boosts your immune system rather than directly killing dividing cells, so side effects are different, often immune-related inflammation rather than hair loss and low counts.
What immune side effects occur?
Rash, diarrhoea, inflammation of the lungs, liver or thyroid, and others. Most are treatable with steroids if caught early, so report symptoms quickly.
How soon will it work?
Responses can take weeks to months. Occasionally tumours appear larger on early scans before shrinking, which your oncologist will interpret.
How long does the course last?
Often up to two years or until the cancer progresses or side effects prevent continuing, depending on the cancer and drug.
