About Endoscopic Third Ventriculostomy (ETV)
Through a small hole in the skull, the surgeon passes a thin endoscope into the ventricles and makes a tiny hole in the floor of the third ventricle. Fluid can then flow to the spaces around the brain, where it is absorbed. It works best where the blockage lies within the ventricular system, as in aqueduct narrowing.
When it works, ETV avoids the need for a shunt and its risks of blockage and infection. Success depends on the cause of hydrocephalus, the patient's age and anatomy. Some people need a shunt later if the opening closes, which can happen months after surgery, so symptoms should be watched.
Plan roughly four days in hospital and a few weeks of recovery. Bring brain scans and previous reports. Because the opening can close, arrange follow-up with a neurosurgeon at home and learn the warning signs, which include headache, vomiting, drowsiness and, in babies, a bulging soft spot or increasing head size.
Usually a good fit if
- Obstructive hydrocephalus, for example from aqueduct narrowing or a tumour blocking fluid flow.
- Patients with a shunt that has failed, if suitable.
- Older children and adults with suitable anatomy.
- Those wishing to avoid long-term shunt hardware.
May need to wait if
- Communicating hydrocephalus where fluid absorption is the problem, in which ETV is less likely to work.
- Very young infants, in whom success rates are lower.
- Anatomy such as a thick or abnormal ventricle floor.
- Active brain infection or uncorrected bleeding disorder.
How to prepare
- Bring MRI and CT scans on disc and notes about any previous shunt or infection.
- Review blood thinners and other medicines with your doctor, who will instruct any changes.
- Expect blood tests and an MRI that checks the floor of the third ventricle and the brain's fluid spaces.
- Tell the team about earlier brain infection, bleeding or radiotherapy.
- Ask about monitoring for several weeks, as signs of failure can return.
Questions to ask your surgeon
- Am I likely to succeed with ETV rather than a shunt, and why?
- How many ETV procedures do you perform each year?
- What signs would show it is failing, and how soon can that happen?
- Can you place a shunt if the ETV fails?
- What follow-up scans do I need?
- Can my local hospital manage an emergency if symptoms return?
Red flags when comparing hospitals
- ETV promised for all types of hydrocephalus.
- No shunt back-up or emergency neurosurgery available.
- No ventricular MRI assessment beforehand.
- No clear plan for follow-up scans and warning signs.
Questions about Endoscopic Third Ventriculostomy (ETV) abroad
Which country is cheapest for Endoscopic Third Ventriculostomy (ETV)?
The lowest typical hospital prices are in India ($11,818), Thailand ($14,190), Turkey ($16,048), out of 9 countries we compare.
How long will I stay in hospital for Endoscopic Third Ventriculostomy (ETV)?

Does ETV need a shunt?
No implant is left. If it fails, a shunt may still be needed later.
When might it fail?
Most failures happen in the first weeks to months, so warning signs must be taken seriously after you go home.

Can I fly home?
Often after a few days to a week if you are well and scans are acceptable. Your surgeon will advise.
