About Deep Endometriosis Excision

Surgery is usually done by keyhole technique with the surgeon removing each nodule and freeing the organs. Depending on location, this can involve shaving the bowel or bladder surface, removing a segment, or repairing the ureter. Detailed MRI or specialised ultrasound beforehand maps the disease so the team can plan the extent of surgery.

Many people have a significant reduction in pain and improved quality of life, but endometriosis can return, and results vary. Outcomes depend on how complete the excision is, the organs involved, whether other pain conditions are present and the surgeon's experience in deep disease. Fertility results depend on age and ovarian reserve.

Plan for about three days in hospital and several weeks of recovery, longer if bowel work was needed. Bring your MRI or ultrasound reports and surgical history. Allow time for pathology results, and arrange hormone treatment and pain follow-up with your own gynaecologist after returning home.

Usually a good fit if

  • Deep infiltrating endometriosis on scan with severe pain, painful periods, painful sex or pain on bowel movements.
  • Symptoms that have not responded to hormonal treatment, or disease causing kidney or bowel obstruction.
  • Endometriosis cysts (endometriomas) or nodules affecting fertility, after specialist counselling.
  • A patient who understands that surgery aims to relieve symptoms, with a high-volume multidisciplinary team.

May need to wait if

  • Mild symptoms that settle with medicine and have no evidence of deep disease on imaging.
  • No mapping scan, because the extent of disease must be known before planning surgery.
  • An unplanned operation by a general gynaecologist without bowel, bladder or urology support available.
  • Pregnancy, active infection or serious health conditions that need optimising first.
How to prepare
  1. Have a pelvic MRI or specialist ultrasound to map deep disease, and consider bowel preparation if bowel surgery is likely; bring all previous reports.
  2. Discuss your hormonal treatment with your doctor and whether to continue it, and follow only the instructions you are given.
  3. Ask your doctor about blood thinners, iron levels and pre-operative blood tests.
  4. Talk about fertility wishes, as this changes the extent of surgery and whether to preserve the ovaries.
  5. Plan for a longer recovery and confirm with the team when it is safe to fly.
Questions to ask your surgeon
  1. How extensive is my disease on the scan, and what will you remove?
  2. Will bowel or bladder surgery be needed, and is a colorectal or urological surgeon in the team?
  3. How many deep endometriosis operations do you do each year?
  4. How will you protect my ovaries and fertility, and will I need a stoma?
  5. What is the chance symptoms return, and what hormonal treatment do I need afterwards?
  6. How is follow-up arranged at home?
Red flags when comparing hospitals
  • No MRI or specialist ultrasound mapping before the operation.
  • No colorectal or urology backup for deep disease involving the bowel or ureters.
  • Promises that surgery will cure endometriosis completely.
  • No fertility or hormonal plan after surgery.

Questions about Deep Endometriosis Excision abroad

Which country is cheapest for Deep Endometriosis Excision?

The lowest typical hospital prices are in India ($5,998), Turkey ($8,268), Saudi Arabia ($14,339), out of 4 countries we compare.

How long will I stay in hospital for Deep Endometriosis Excision?

Most patients spend about 3 nights in hospital. Full recovery takes about 45 days; your surgeon will tell you when you are fit to fly home.

Will the endometriosis come back?

Recurrence is possible, even after thorough excision, especially if surgery leaves disease behind. Hormonal treatment afterward may reduce the risk.

Will surgery help my pain?

Many women have significant relief, but not all. Results depend on disease extent and how complete the excision is.

Will I need bowel surgery?

Only if deep disease invades the bowel wall. The scan and surgeon's plan will show whether shaving, disc excision or resection is likely.

Can I get pregnant afterwards?

Fertility can improve in some women, but this depends on age, ovarian reserve and disease. Discuss with a fertility specialist.
