About Open Myomectomy
Under general anaesthesia, the surgeon opens the lower abdomen with a horizontal cut, makes incisions in the womb wall, removes each fibroid and rebuilds the muscle in layers. Open access allows the surgeon to feel the womb and remove many or large fibroids. Medication or a tourniquet may be used to limit blood loss.
Heavy bleeding and pressure symptoms usually improve, and many women conceive afterwards. Fibroids can regrow, and blood loss is a main risk that is prepared for by treating anaemia first. Outcomes depend on the number, size and position of fibroids, your age, and the surgeon's experience. A later delivery may need to be by caesarean.
Plan for about four days in hospital and up to six weeks of recovery. Have your haemoglobin checked and treated in advance, and bring MRI or ultrasound reports. Discuss when to try to conceive, and arrange follow-up with your own gynaecologist.
Usually a good fit if
- Large or numerous fibroids not suitable for keyhole surgery, causing heavy bleeding, pressure or pain.
- A woman who wants to keep her womb, especially for future pregnancy.
- Fibroids in difficult positions, such as deep in the wall or near the broad ligament.
- Fibroids that distort the womb and are affecting fertility.
May need to wait if
- Suspected sarcoma or other uterine cancer, which needs a different cancer approach.
- Severe untreated anaemia or bleeding disorders needing correction.
- Pregnancy or active pelvic infection.
- Serious heart or lung disease that makes major surgery unsafe until optimised.
How to prepare
- Have a pelvic MRI or ultrasound to map the size and position of fibroids, and bring reports.
- Ask whether hormone treatment is advised beforehand to shrink fibroids or correct anaemia, and follow only your doctor's instructions.
- Treat anaemia with iron, and arrange blood availability in case of heavy bleeding; ask the team about this.
- Ask your doctor about blood thinners and aspirin, and follow only the instructions you are given.
- Plan a longer stay, confirm when it is safe to fly, and discuss future pregnancy and delivery plans.
Questions to ask your surgeon
- Why do you advise open rather than keyhole surgery for my fibroids?
- What blood loss do you expect, and what measures reduce it?
- How many open myomectomies do you perform each year?
- How will the womb be repaired, and will I need a caesarean later?
- When can I try to conceive after the operation?
- What is the chance of adhesions or fibroid recurrence?
Red flags when comparing hospitals
- No scan mapping or discussion of surgical approach before surgery.
- No blood-availability planning for what can be a bloody operation.
- Hysterectomy suggested without discussing myomectomy if you want to keep your womb.
- No advice on timing of pregnancy and delivery.
Questions about Open Myomectomy abroad
Which country is cheapest for Open Myomectomy?
The lowest typical hospital prices are in India ($6,898), Thailand ($7,815), Turkey ($9,498), out of 9 countries we compare.
How long will I stay in hospital for Open Myomectomy?
Most patients spend about 4 nights in hospital. Full recovery takes about 42 days; your surgeon will tell you when you are fit to fly home.
Will I need a blood transfusion?
Transfusion is possible with large fibroids, and surgeons plan for it. Correcting anaemia beforehand lowers the chance. Ask your surgeon about their experience.
Can fibroids come back?
New fibroids can grow over time, especially in younger women or after surgery for many fibroids.
When can I try to get pregnant?
Usually after about 3 to 6 months, depending on how deep the womb was cut. Your surgeon will advise.
Will I need a caesarean?
Often yes if the cavity was entered or fibroids were deep. Your surgeon will document this in your operation notes.
