About Endometrial Ablation

Under general anaesthesia or sedation, a device is passed through the cervix and destroys the endometrium using heat, radiofrequency energy, a hot fluid balloon or freezing. A hysteroscopic version uses a camera and a resecting loop. A scan or biopsy beforehand rules out polyps, fibroids or other causes.

Many women have much lighter periods and some stop bleeding altogether, though periods can return, and a minority later need a hysterectomy. Pregnancy after ablation is dangerous, so reliable contraception is required. Results depend on the cause of bleeding, womb size and shape, the presence of fibroids, and your age.

This is generally a day case, and most people resume normal activity within a week. Expect watery or bloody discharge and period-like cramps for a short time. Bring recent ultrasound and endometrial biopsy results, and arrange with your own doctor to review bleeding and contraception.

Usually a good fit if

  • Heavy menstrual bleeding without cancer, with normal womb-lining biopsy, that has not responded to medicines or a hormone coil.
  • Completed childbearing and no wish for future pregnancy.
  • A womb of normal size and shape on ultrasound, or one with small fibroids that the device can treat.
  • A woman who wants to avoid hysterectomy, accepting it may not stop bleeding completely.

May need to wait if

  • A wish to get pregnant in the future, since ablation is not meant to be reversed.
  • Suspected or confirmed womb cancer, or abnormal cells in the lining.
  • A large womb or large fibroids in the cavity, or a thin, damaged womb wall from earlier surgery such as a classical caesarean.
  • Pregnancy, active pelvic infection, or severe period pain better managed by other treatment.
How to prepare
  1. Have a pelvic ultrasound and a biopsy of the womb lining to exclude abnormal cells, plus a smear if due, and bring reports.
  2. Make sure you have reliable contraception, because pregnancy after ablation is dangerous; discuss this with your doctor.
  3. Ask your doctor about hormones, blood thinners and aspirin, and follow only the instructions you are given.
  4. Have a blood test for anaemia and discuss iron treatment if you have lost a lot of blood.
  5. Fast as instructed before anaesthesia, and bring sanitary pads for the journey home.
Questions to ask your surgeon
  1. Which type of ablation device do you advise for my womb shape, and why?
  2. Do I need a womb-lining biopsy and hysteroscopy first?
  3. What are the chances my periods will stop, or reduce, and the chance I will need further treatment?
  4. How many ablations do you perform each year?
  5. What contraception will I need, and can I still have a smear?
  6. What follow-up do I need at home if bleeding returns?
Red flags when comparing hospitals
  • Ablation offered without first excluding cancer of the womb lining by biopsy or hysteroscopy.
  • Promises that periods will always stop completely.
  • No discussion of contraception or of the risks if pregnancy occurs afterward.
  • No mention that some women later need a hysterectomy.
