About DBS for Essential Tremor
An electrode is guided to the ventral intermediate nucleus of the thalamus using MRI and often with the patient awake to test tremor response. A wire runs under the skin to a pulse generator in the chest. Once stimulation is switched on, the settings are adjusted over several visits. Focused ultrasound and radiosurgery are alternative lesioning options.
Most people see a clear reduction in tremor of the hand and arm, which can make eating, writing and drinking easier. The benefit depends on accurate placement and programming, and some tremor can return over the years. Stimulation may cause tingling, speech slurring or unsteadiness, which can usually be reduced by changing the settings.
Plan a hospital stay of around five days and a further period near the team for the first programming sessions. Bring your neurology records and a video of the tremor if possible. Find a clinic at home that can adjust the device and replace the battery, and keep your device card with you.
Usually a good fit if
- Disabling essential tremor of the hands that has not responded to at least two medicines.
- Tremor affecting eating, writing or work.
- Good general health and clear thinking.
- Patients able to attend programming sessions.
May need to wait if
- Mild tremor that is controlled with medicine.
- Significant memory problems or dementia.
- Severe untreated psychiatric illness, active infection or bleeding disorder.
- Inability to have regular programming visits.
How to prepare
- Bring MRI images and a list of tremor medicines tried, such as propranolol or primidone, with doses and effects.
- Review all medicines with your neurologist, who will say what to continue.
- Expect MRI, movement-disorder assessment, and memory and mood screening.
- Have any infections, including dental, treated before surgery.
- Plan follow-up programming visits, since adjustment continues for months.
Questions to ask your surgeon
- Is DBS or focused ultrasound a better option for me?
- Will one or both sides be treated, and why?
- What is the typical tremor reduction and what is the risk to speech and balance?
- How many DBS operations do you perform each year?
- Who will programme the device when I go home?
- What is the battery life or recharging plan?
Red flags when comparing hospitals
- No movement-disorder specialist programming the device.
- Offering DBS for tremors that have not had a trial of medicines.
- No clear follow-up or remote programming arrangement.
- Promise of complete lifelong tremor control.
