About DBS for Dystonia
Thin electrodes are placed in a target deep in the brain, usually the globus pallidus, using MRI-guided frames or robotic navigation. They are connected under the skin to a pulse generator in the chest. Stimulation is switched on and tuned over several visits. Some centres do the implant in stages, and the procedure is often done with only local anaesthesia and sedation.
Improvement can be large in some forms of dystonia, particularly inherited or generalised types, but it may build slowly over weeks to months. Results depend on the type and cause of dystonia, how long it has been present and careful programming. It does not cure the condition, and settings usually need adjusting over time.
Plan for about five days in hospital, then a stay close to the team while the device is first programmed. Programming and battery checks continue for years, so confirm that a trained clinic near home can take over. Carry your device identification card, as it affects MRI scans and airport security.
Usually a good fit if
- Generalised or segmental dystonia, including DYT1 dystonia, not controlled by medicines.
- Cervical dystonia that no longer responds well to botulinum toxin.
- Patients with disabling symptoms and realistic expectations of gradual improvement.
- Those who can return for programming.
May need to wait if
- Dystonia caused by some structural brain lesions, which respond less well.
- Severe untreated depression or significant cognitive decline.
- Active infection or inability to attend programming visits.
- Medical conditions that rule out MRI or surgery.
How to prepare
- Bring MRI images, genetic results if known, and a list of medicines and botulinum toxin treatments tried.
- Review all medicines with your neurologist, who will say what to continue before surgery.
- Expect a movement-disorder assessment, MRI, and cognitive and mood screening.
- Have your teeth and any infections treated before implanting hardware.
- Discuss programming plans, as you need repeated adjustment visits after surgery.
Questions to ask your surgeon
- Which brain target and which device will you use, and is it rechargeable?
- How soon is stimulation turned on, and how long until benefit appears?
- How many DBS operations for dystonia do you perform each year?
- What are the risks of bleeding, infection, speech and swallowing change?
- Who will programme the device after I go home?
- What are the MRI and airport security rules for my device?
Red flags when comparing hospitals
- No movement-disorder neurologist on the DBS team.
- No clear plan for programming and follow-up after you return home.
- Promise of full cure.
- No explanation of the battery type and replacement.
