About David Procedure (Valve-Sparing Root)

Under general anaesthesia and with a heart-lung machine, the surgeon removes the enlarged root and sews the patient's own valve inside a fabric tube graft, then reattaches the coronary arteries. Because the native valve is kept, there is generally no need for lifelong blood thinners. The operation is technically demanding and is done mainly in centres with experience.

Results are good in carefully selected patients, with many people keeping their own valve for years, but the valve can leak later and may need further treatment. Outcomes depend on the shape of the valve leaflets, any connective tissue condition such as Marfan syndrome, age and the surgeon's experience with this specific operation.

Plan to stay near the hospital for around ten days to two weeks and avoid heavy lifting for several weeks. Bring your scans and genetic or family history information. Arrange regular echocardiograms at home, since the preserved valve needs lifelong monitoring, and tell your local team exactly which operation you had.

Usually a good fit if

  • Aortic root aneurysm, usually around 5.0 centimetres or more, or lower in certain genetic conditions.
  • Normal or only mildly leaking aortic valve leaflets that can be preserved.
  • Younger patients who want to avoid lifelong warfarin from a mechanical valve.
  • Care at a centre with substantial valve-sparing experience.

May need to wait if

  • Thick, calcified or damaged leaflets, or severe valve disease unsuitable for sparing.
  • Active endocarditis of the aortic valve.
  • Severe heart, lung or kidney disease increasing surgical risk too high.
  • Tissue quality problems that make long-term durability uncertain.
How to prepare
  1. Bring CT or MR angiograms and a detailed echocardiogram describing the valve, root diameter and degree of leakage.
  2. Have blood tests, coronary angiography or CT, lung and kidney assessments.
  3. Review blood thinners, blood-pressure drugs and diabetes tablets with your doctor, who will tell you what to adjust.
  4. Tell the team about Marfan syndrome, bicuspid valve or family aortic disease, which influence the plan.
  5. Plan a long stay near the hospital with help during the first weeks.
Questions to ask your surgeon
  1. Is my valve suitable for sparing, and what is your backup plan if it is not?
  2. How many David or valve-sparing root operations do you perform each year?
  3. What is your rate of later valve leakage requiring reoperation?
  4. Will you use the reimplantation technique, and why?
  5. Will I need any blood thinners afterwards?
  6. What imaging will I need for the rest of my life?
Red flags when comparing hospitals
  • Surgeon has little valve-sparing experience and cannot quote their own results.
  • No intraoperative echo available to check the repaired valve.
  • No agreement on a backup Bentall procedure.
  • Promises that the valve will last forever.
  • No cardiac intensive care on site.

Questions about David Procedure (Valve-Sparing Root) abroad

Which country is cheapest for David Procedure (Valve-Sparing Root)?

The lowest typical hospital prices are in India ($20,653), Thailand ($25,430), Turkey ($29,590), out of 9 countries we compare.

How long will I stay in hospital for David Procedure (Valve-Sparing Root)?

Most patients spend about 9 nights in hospital. Full recovery takes about 56 days; your surgeon will tell you when you are fit to fly home.

Why choose the David procedure over a Bentall?

It keeps your own valve, which can avoid lifelong warfarin and the risks of a prosthetic valve, if the valve is healthy.

Is it a major operation?

Yes. It is complex open-heart surgery with a hospital stay of about a week to ten days and recovery over several months.

Could the valve fail later?

The preserved valve can develop leakage over time in a small number of patients, so regular echocardiograms are required.

Is it suitable for Marfan syndrome?

Often yes, and it is commonly performed in these patients, but suitability depends on valve quality and surgeon experience.
