About Brain Tumor Surgery

The surgeon opens a section of skull and removes as much tumour as can be done safely, guided by MRI navigation, a microscope and sometimes monitoring or an awake approach near speech and movement areas. Where full removal is risky, a biopsy or partial removal may be done. Tissue is examined to identify the tumour type, which shapes what treatment follows.

What surgery achieves depends on the tumour type, its grade and position, and the extent that can be safely removed. Some benign tumours are cured by complete removal, while others need radiotherapy or chemotherapy afterwards. Recovery of strength, speech or vision depends on the area involved and on swelling around the tumour, which usually eases over weeks.

Expect around ten days in hospital and a recovery of several weeks to a few months. Bring all scans on disc, previous reports and a list of medicines. Because pathology results guide the next steps, arrange for oncology and neurosurgical follow-up at home before you travel, and have a companion with you.

Usually a good fit if

  • A tumour accessible to surgery, for diagnosis or removal.
  • Tumour causing pressure symptoms, seizures or weakness that surgery may relieve.
  • Patients fit enough for anaesthesia and for follow-on treatment.
  • Selected single metastases in suitable locations.

May need to wait if

  • Tumours deep in vital regions where risk may outweigh benefit.
  • Widespread disease where surgery will not change the outlook.
  • Severe illness making anaesthesia unsafe.
  • Uncorrected bleeding disorder or active infection.
How to prepare
  1. Bring all MRI images on disc, plus any pathology, biopsy or scan reports.
  2. Review steroids, anti-epileptic medicine and blood thinners with your doctor, who will instruct any changes.
  3. Expect blood tests, heart checks and sometimes extra functional MRI or tractography.
  4. Ask which tests are needed before travel, and whether steroids are needed to control swelling.
  5. Arrange a companion and plan for the pathology result, which may take several days.
Questions to ask your surgeon
  1. What type of tumour do you think this is, and what is the goal: biopsy, partial removal or full removal?
  2. Will you use image guidance, awake mapping or fluorescence?
  3. What risks relate to this tumour's location?
  4. When will I get the pathology, and who plans radiotherapy or chemotherapy?
  5. How many operations like mine do you do each year?
  6. What scans and treatment do I need at home?
Red flags when comparing hospitals
  • No MRI-based planning or image guidance available.
  • No pathology service or neuro-oncology team to plan next steps.
  • No intensive care for post-operative monitoring.
  • Promises of cure before tissue diagnosis.
