About Autologous Bone Marrow Transplant

Stem cells are first collected from your blood after medicine boosts their numbers, then frozen. You then receive high-dose chemotherapy, which destroys cancer cells and also your marrow, and the stored cells are returned through a vein. They restore blood production in around ten to fourteen days, during which you are very vulnerable to infection.

Because the cells are your own, there is no risk of graft-versus-host disease, and recovery is generally quicker than after a donor transplant. The benefit depends on the disease, how well it responded to earlier treatment and your general health. It often controls disease for a period rather than curing it.

Plan roughly three weeks in hospital and a further few months of recovery near the team, with infection precautions at home. Bring your treatment history and response reports. Arrange for a haematologist at home to continue maintenance therapy or monitoring, and have a caregiver, because fatigue can last for months.

Usually a good fit if

  • Multiple myeloma in patients fit for high-dose therapy, after initial treatment.
  • Relapsed or high-risk lymphoma that responds to salvage chemotherapy.
  • Selected autoimmune conditions or germ cell tumours as specialist care directs.
  • Patients fit enough to withstand high-dose chemotherapy.

May need to wait if

  • Disease not responding to chemotherapy, as advised by the transplant team.
  • Severe heart, lung, liver or kidney disease.
  • Failure to collect enough stem cells.
  • Active infection or very poor performance status.
How to prepare
  1. Bring your diagnosis, pathology, scan and treatment history, plus recent bone marrow results.
  2. Do not stop any medicine unless your doctor instructs you, and review supplements with the team.
  3. Expect heart, lung and kidney tests, infection screening and a dental check.
  4. Plan for a central line and stem cell collection before the transplant.
  5. Arrange a carer and accommodation for the stay and early recovery.
Questions to ask your surgeon
  1. Why autologous rather than allogeneic transplant for my disease?
  2. How many stem cells will be collected, and what if collection fails?
  3. Which high-dose regimen will be used, and what are its side effects?
  4. How many autologous transplants does your centre do each year?
  5. What is the chance of remission, and is maintenance therapy planned?
  6. What follow-up can happen at home?
Red flags when comparing hospitals
  • No stem cell collection and storage facility.
  • No isolation unit or specialised nursing.
  • Centre does not share its transplant experience.
  • No plan for maintenance therapy and follow-up.

The lowest typical hospital prices are in India ($42,453), Thailand ($52,215), Turkey ($58,825), out of 9 countries we compare.

How long will I stay in hospital for Autologous Bone Marrow Transplant?

Most patients spend about 21 nights in hospital. Full recovery takes about 270 days; your surgeon will tell you when you are fit to fly home.

Is it a cure?

For myeloma it controls disease for a period rather than curing. For some lymphomas it can cure. Your doctor will explain the aim.
