Discharge summary

A document summarizing the hospital admission, treatment given, and follow-up plan.

The discharge summary should include: diagnosis, procedures done, complications, medications, follow-up appointments, activity restrictions, warning signs, and contact information for the operating team.

What this means for you: insist on a complete discharge summary in English (and a copy in the local language if relevant). Your home physician needs this to continue care.
