À propos de Pediatric Pyeloplasty
Under general anaesthesia the surgeon cuts out the narrowed section of the pelvi-ureteric junction and rejoins the healthy ureter to the kidney pelvis (the Anderson-Hynes technique). It can be done through an open flank incision, with keyhole instruments or with a robot. A small tube (stent) often stays inside the ureter for a few weeks to keep it open while it heals.
The aim is to improve drainage and stop further kidney damage, and most children do well. Outcomes depend on the amount of kidney function there was before surgery, the child's age, the technique and the surgeon's experience. Follow-up ultrasound or a kidney scan after some months confirms that the blockage has gone, and a few children need a second procedure.
Plan to stay near the hospital for the first days and expect a stent-removal procedure some weeks later, which may need a second short anaesthetic. Ask whether it can be done at home. Bring earlier scans and kidney function results, and arrange follow-up ultrasounds locally with a paediatric urologist or nephrologist.
Généralement adapté si
- Significant pelvi-ureteric junction obstruction shown on ultrasound and a renogram, particularly with falling kidney function.
- Repeated infections, pain or stones related to the blocked kidney.
- Increasing kidney swelling on follow-up scans in a baby with antenatal hydronephrosis.
- A child fit for general anaesthesia with a team experienced in paediatric reconstruction.
Il peut falloir attendre si
- Mild, stable dilation with good kidney function may be safely watched without surgery.
- Active urinary infection should be treated and cleared first.
- A kidney with almost no function may need a different plan, which the specialist will discuss.
- Uncontrolled bleeding or anaesthetic risks require optimisation beforehand.
Comment se préparer
- Bring all kidney ultrasounds, renogram (MAG3) reports, and any urine culture results so the surgeon can confirm the obstruction.
- Make sure any urine infection is treated before surgery; your doctor may suggest preventive antibiotics.
- Report allergies, previous anaesthetics and all medicines; changes only on the doctor's instruction.
- Follow fasting instructions for the child's age, and arrange a parent to stay in hospital.
- Plan to return for the stent-removal procedure, which needs a short anaesthetic, or stay long enough to arrange it.
Questions à poser à votre chirurgien
- Does my child need surgery now, or can the kidney be watched with scans?
- Which approach do you recommend, open, laparoscopic or robotic, and why?
- Will a stent be placed, how long will it stay, and how is it removed?
- How many paediatric pyeloplasties do you perform each year, and what is your re-operation rate?
- What kidney function improvement can we realistically expect, and how will you measure it?
- Who will repeat the ultrasound and renogram at home, and when?
Signaux d'alerte à surveiller en comparant les hôpitaux
- Surgery is recommended without a renogram or clear evidence of obstruction.
- The team has limited experience operating on children's kidneys or ureters.
- No explanation about stents, their removal and the need for a second anaesthetic.
- No plan for follow-up scans to confirm drainage.
