À propos de Pediatric Liver Resection
Surgeons plan the operation from detailed CT or MRI scans that show how the tumour sits against the liver's blood vessels and bile ducts. Under general anaesthesia the affected segment or lobe is removed through an open abdominal incision, often after chemotherapy has shrunk the tumour. In some children where a resection is not possible, a liver transplant is considered instead.
A complete removal gives the best chance of cure for tumours such as hepatoblastoma. Results depend on the type and stage of the tumour, how well it responded to chemotherapy, whether the margins are clear and the amount of healthy liver remains. The liver regenerates over weeks to months. Teams with regular paediatric liver surgery experience handle bleeding and bile leaks better.
Plan for a longer stay in hospital, with intensive care monitoring at first, and then a period near the centre for checks on blood tests and wound healing. Chemotherapy often continues afterwards, so agree who delivers it at home. Bring all scans, biopsy reports and treatment records so the team can see the full history.
Généralement adapté si
- A resectable liver tumour or lesion confirmed on imaging and, usually, biopsy, such as hepatoblastoma after any planned chemotherapy.
- Enough healthy liver would remain, with usable blood and bile drainage, after the planned resection.
- A multidisciplinary team has recommended surgery, often after chemotherapy shrinks the tumour.
- Your child is fit enough for major surgery and intensive care.
Il peut falloir attendre si
- Widespread tumour that cannot be removed safely may need chemotherapy first or a transplant assessment.
- Poor liver function or uncontrolled clotting problems need optimising before surgery.
- Active infection or sepsis should be treated before an elective resection.
- Spread to other organs may change the order of treatment; your oncology team will advise.
Comment se préparer
- Bring all scans, biopsy reports, tumour-marker results (such as alpha-fetoprotein) and chemotherapy records so the team can plan the resection.
- Expect blood tests, clotting studies, a detailed CT or MRI and sometimes blood group cross-matching before surgery.
- Have your child's nutrition, anaemia and any infection reviewed; medicine changes only on the doctor's instruction.
- Follow exact fasting instructions and make sure the hospital has a paediatric intensive care unit and blood bank.
- Plan a stay near the hospital for several weeks, and arrange for a parent or carer to be with your child.
Questions à poser à votre chirurgien
- Is my child's tumour resectable, and what percentage of the liver will you remove?
- Will chemotherapy come before or after surgery, and who will deliver it?
- How many paediatric liver resections does your unit perform each year, and is a transplant team available?
- What are the main risks, such as bleeding or bile leak, and how are they managed?
- How long will my child be in intensive care and in hospital overall?
- Who will arrange tumour-marker tests, scans and chemotherapy follow-up when we return home?
Signaux d'alerte à surveiller en comparant les hôpitaux
- No paediatric intensive care unit, paediatric anaesthetists or blood bank on site.
- Surgery proposed without a tumour board discussion or a clear imaging plan.
- The unit does not usually operate on children's livers or has no transplant link for complex cases.
- No arrangement for the oncology treatment that often goes with the operation.
