À propos de PCNL (Percutaneous Nephrolithotomy)
Under general anaesthesia the surgeon punctures the kidney, widens the track and passes a rigid telescope in. A laser, ultrasonic or pneumatic probe breaks the stone and the pieces are removed directly. The patient may lie face down or on their back. A tube (nephrostomy) in the kidney or a ureteric stent is sometimes left for a day or more.
PCNL has the highest chance of clearing big stones in a single operation, and staghorn stones sometimes need more than one. Outcomes depend on stone size, number and composition, kidney anatomy, a treated urine infection and the surgeon's experience. Bleeding is the main risk and rarely needs a blood transfusion. A CT scan afterwards checks for fragments left behind.
Expect about three nights in hospital, with a drain or tube sometimes removed before you leave. Allow time nearby for the wound check, and wait about a week or two before a long flight. Bring your CT scan, urine culture and a list of blood-thinning medicines, which must be managed carefully, and arrange a follow-up scan at home.
Généralement adapté si
- Kidney stones larger than about 2 cm, including staghorn stones.
- Stones that have not cleared with shock waves or ureteroscopy.
- Stones in an abnormal kidney or with difficult anatomy where other routes are unsuitable.
- A normal clotting profile and no active infection.
Il peut falloir attendre si
- Active urinary infection must be treated first.
- Uncorrected bleeding disorders, or blood thinners that cannot be paused, need a plan.
- Severe spine deformity or lung disease may make positioning unsafe.
- Uncontrolled heart or other conditions need optimisation.
Comment se préparer
- Bring your CT scan, kidney function tests and urine culture; infection must be treated before surgery.
- Tell your doctor about blood thinners and other medicines; changes only on the doctor's instruction.
- Expect blood tests, blood group cross-match and an anaesthetic review.
- Follow fasting instructions and arrange help at home for the first week.
- Plan to stay near the hospital until tubes are out and you are fit to travel.
Questions à poser à votre chirurgien
- Is PCNL needed for my stone, or could RIRS or ESWL do the job?
- How many PCNLs do you perform each year, and what is your stone-free and transfusion rate?
- Will I need a nephrostomy tube, and for how long?
- How do you manage bleeding, infection and injury to nearby organs?
- Do you plan a single session or staged procedures for a large stone?
- Who will repeat a scan and give prevention advice at home?
Signaux d'alerte à surveiller en comparant les hôpitaux
- PCNL offered without CT imaging or a clear plan for the puncture.
- No blood bank, interventional radiology or intensive care backup.
- Complication rates and stone-free results are not shared.
- No plan for a nephrostomy tube, stent or follow-up scan.
